ENCLOSURE V0GTLE ELECTRIC GENERATING PLANT - UNIT 1 NRC NOTICE OF VIOLATION 50-424/90-05-01 AND GPC RESPONSE
" Technical Specification 3.4.1.4.2.c, requires that, in Mode 5 with reactor coolant loops not filled, Reactor Makeup Water Storage Tank discharge valves, 1-1208-U4-175,176,177, and 183 be closed and secured in position.'
Contrary to the above, on February 26,-1990, with Unit _1 in Mode 5 and RCS level I
at 195 feet 5 inches, valve 1-1208-U4-176 was closed but was not mechanically l
secured in the closed position.

The violation occurred as-stated.
Reason for the Violation:
As stated in the Notice ~of Violation, Technical Specification 3.4.1.4.2.c r
requires the-subject valve to be-closed and secured in-position.
Similarly, Technical Specification (TS) 3.6.1.7 requires the 24-inch containment purge supply and exhaust isolation. valves.to be closed and sealed' closed. The bases t
for TS 3.6.1.7 state that sealed closed isolation. valves are isolation' valves under administrative control to assure they cannot be inadvertently opened. The administrative controls, which constituted the locked valve policy, permitted clearance " hold tags" to be utilized to-secure small vakes which cannot feasibly be locked. However, Surveillance Requirement 4.4.1.4.2.2 requires the-subject valve to be secured in position:by mechanical stops. Therefore, the administrative controls were less than adequate which resulted in the violation.
Corrective Steos Which Have Been Taken and the Results Achieved:-
-The subject valve was wired in the closed position to provide a " mechanical stop" as required by Surveillance Requirement 4.4.1.4.2.2.
Corrective Steos Which Will Be Taken To Avoid Further Violations:-
To preclude any further violations, the following corrective steps will be-taken:-
o The-locked valve procedure will be revised by 9-15-90, to eliminate utilization of a " hold tag" on valves that are required by the Technical Specifications to be secured in position.
