Haddam Neck Plant Notification of Missed Commitment The purpose of this letter is for Connecticut Yankee Atomic Power Company (CYAPCO) to notify the Nuclear Regulatory Commission (NRC) that commitments contained in the CYAPCO letter to the NRC, dated August 21, 1996,' were not completed within the planned schedule. The August 21,1996 letter was a reply to a notice of violation and notice of deviation contained in NRC Inspection Report No. 96-04, dated July 22,1996. The violation concerned a safety evaluation performed to move fuel assemblies in the spent fuel pool using an extension sling on the fuel handling crane. The deviation concerned use of a lifting device that had not been inspected according to the periodic inspection program.

The following were the CYAPCO commitments made within the referenced letter.

B15855-1:

An approved safety evaluation and a change to the UFSAR is being processed to make the licensing basis documents consistent, specifying that fuel within the spent fuel pool will be moved with 7 feet minimum of submergence. This UFSAR change will be processed by December 31, 1996 and be included in Revision 10 to the UFSAR.

B15855-2:

A comprehensive review of the safety evaluation which was performed to initiate this fuel transfer evolution will be completed. This review will be completed by October 31,1996.

B15855-3:

ACM 2.2-9 " Control of Crane Operations" is being revised to include physical qualification of the crane operators. The Maintenance 4 (1)

Department will revise the OJT (On-The-Job-Training) Guide to require verification of physical qualifications. These changes will be completed by September 30,1996.

B15855-4:

WCM 2.2-7, "PAB/ Pipe Trench Floor Block Lifting Procedure" is being revised to designate the use of standard rigging components instead of i

the lift rig when lifting the floor blocks. This revision will be completed by l

October 31,1996.

All commitments contained within the August 21,1996 letter have now been completed.

'A detailed status of these commitments is provided below.

B15855-1 The change request for the UFSAR to specify that fuel within the spent

fuel pool will be moved with a minimum of 7 feet of submergence, was completed on December 19, 1996 and will be included in the 1997 UFSAR update.

B15855-2 The commitment to complete a comprehensive review of the safety evaluation which was performed to initiate the subject fuel transfer evolution was scheduled to be completed by October 31,1996. During the preparation for defueling, it was discovered that the comprehensive review had not been completed. immediate action was taken and the review was completed on November 8,1996. This review concluded that sufficient vendor information was not available to have-identified the potential problem.

B15855-3 This commitment contained two action items. The first was to revise procedure ACM 2.2-9, " Control of Crane Operations", to include physical qualification of the crane operators.

This procedure was revised on August 28,1996, within the scheduled date. The second action item was to revise the On-The-Job-Training (OJT) Guides to require verification of physical qualifications.

The related OJT Guides were updated by September 30,1996, except for the OJT Guide for Polar Crane Operator.

The revision to this guide was completed on November 9,1996.

It was verified that the polar crane operators who had been performing work since August 21,1996 were physically qualified.

B15855-4:

WCM 2.2-7,"PAB/ Pipe Trench Floor Block Lifting Procedure" was revised on October 22, 1996 to designate the use of rigging equipment rather than a special lift device when lifting the floor blocks. This action was completed within the scheduled time.

A review of the commitments contained in the reference letter concluded that there was no adverse impact due to the delay in completing two of the action items. In addition, a review of all other 1996 commitments was conducted with no other problems identified.
