Letter from J. H. Harner, Indiana Michigan Power Company, to J. Sanders, Michigan Department of Environmental Quality, '.'Donald C. Cook Nuclear Plant, Groundwater Permit No. GW1810102," dated April 18, 2016.

By the referenced letter, Indiana Michigan Power Company, the licensee for Donald C. Cook Nuclear Plant (CNP) Units 1 and 2, submitted to the Michigan Department of Environmental Quality, a notification of a spill resulting from a failed transformer at CNP.

The cause of the oil release from the containment system was a failure of a seal between the composite wall of the containment and the concrete firewall foundation. The transformer holding capacity is 30, 119 gallons. As of April 15, 2016, approximately 17,000 gallons of oil had been recovered or accounted for.

Clean up efforts are ongoing. No oil was discharged to the surface water. This spill does not pose a threat to the environment or public health and safety.

The referenced letter is provided as an enclosure to this letter in accordance with CNP's Nonradiological Environmental Protection Plan, Section 5.4.2.

On April 8, 2016, a transformer failed in the 765 KV Switchyard at the Cook Nuc1ear Plant The transformer holding capacity is 30,119 gallons. As of April 15, approximately 17,000 gallons of oil had been recovered and or accounted for. Cleanup efforts are ongoing. At this time, free product has been collected and stored in temporary frac tanks on the site awaiting proper disposal. As of April 15, approximately 135 cubi.c yards of oil saturated soil has been removed from the spill vicinjty. Cleanup at the site continues.. No oil was discharged to the surface water. This spill does not pose a threat to the environment or public health or safety.

The transformer and the system in the vicinity was de-energized which allowed the recovery efforts to begin. Crews worked 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> a day for the first three days containing the spill until all free. product was recovered. Ongoing excavation of contaminated soils continues. The remaining oil in the failed transformer will be pumped out and properly disposed of once it is safe to proceed. We will provide notification to your office when cleanup efforts have been completed.

An investigation of the transformer spill containments is underway to determine the cause of the failure to contain the transformer contents.
