Cook Nuclear Plant Units 1 and 2 NOTIFICATION OF GROUNDWATER PERMIT EXCEEDANCE FOR SODIUM AND CHLORIDE In a letter dated August 5, 2014, Indiana Michigan Power Company (I&M), the licensee for Donald C. Cook Nuclear Plant (CNP) Units 1 and 2, submitted a notification of noncompliance for Sodium and Chloride concentrations at Monitoring Well 19, for Groundwater Permit GW1 810102, to the Michigan Department of Environmental Quality (MDEQ). On July 31, 2014, the Sodium concentration was confirmed to be 482 mg/I, which exceeds the permit limit of 120 mg/l. The Chloride concentration was confirmed to be 821 mg/I, which exceeds the permit limit of 250 mg/I. This event is still being evaluated. I&M resampled this monitoring well on August 1, 2014 and submitted the sample for lab analysis.
I&M will submit a follow-up report to the MDEQ within 60 days with the results of our evaluation. This report will subsequently be provided to the Nuclear Regulatory Commission.
A copy of the notification is provided, as an enclosure to this letter, in accordance with CNP's Nonradiological Environmental Protection Plan, Section 5.4.2.

Dear Mr. Sanders:
This notification is made pursuant to Cook Nuclear Plant's Groundwater Permit GW 1810102, Part I.ll.b),
Compliance Requirements.
On July 31, 2014 we became aware of a noncompliance for Sodium and Chloride concentrations at monitoring well 19.
The Chloride concentration was confirmed to be 821 mg/l, which exceeds the permit limit of 250 mg/l.
The Sodium concentration was confirmed to be 482 mg/l, which exceeds the permit limit of 120 mg/l.
Well 19 is located approximately 1200 feet North of the plant's rapid infiltration basin.
Upgradient wells and other monitoring wells are within permit limits from this sampling period.
As required by Part I.ll.c),
we have scheduled a resample of this monitoring well within the next 14 days (due date August 13, 2014).
As required by Part I.ll.d),
we will submit a written report to the Kalamazoo Office within 60 days (due date Sept 28, 2014).
