Cook Nuclear Plant Units 1 and 2 NOTIFICATION OF NONCOMPLIANCE FOR SANITARY SEWAGE (OUTFALL OOE) WEEKLY SAMPLING ANALYSIS By letter dated August 23, 2012, Indiana Michigan Power Company (I&M), the licensee for Donald C. Cook Nuclear Plant (CNP) Units 1 and 2, submitted a notification of noncompliance for the weekly analysis requirement for Sanitary Sewage (Outfall OOE), to the Michigan Department of Environmental Quality (MDEQ). This analysis is required in accordance with CNP's Groundwater Permit No. GW 1810102 Part 1.2, "Effluent Limitations: Sanitary Sewage (Outfall OOE)."

During the week of August 12, 2012, the samples for ammonia, nitrite, nitrate, total phosphorus, and biochemical oxygen demand (BOD5) were obtained; however, the analyses were missed. The regular operator was called away from work for personal reasons. The sample analyses was missed by the back up operator, who did not understand the full requirements.

By letter.dated September 14, 2012, I&M submitted a letter to the Michigan Department of Environmental Quality, regarding the August 23, 2012, notification. Outfall OOE was resampled the week of August 19, 2012, with all analyses completed. Samples obtained the previous week and the week following the missed analyses showed no abnormal data. There was no impact to the surrounding groundwater from this event. The following corrective actions have been taken to prevent recurrence. A sample package containing sample bottles, preservatives, chain of custody, and shipping paperwork has been prepared and stored at the wastewater lab. Instructions have been posted in the lab to ensure that effluent samples are collected and analyzed onsite or sent to a contract laboratory by replacement operators. No further corrective actions are necessary for this issue. provides the I&M notification to MDEQ dated August 23, 2012. Enclosure 2 provides the I&M notification to MDEQ dated September 14, 2012. These notifications are provided, as enclosures to this letter, which is being sent in accordance with the CNP Environmental Technical Specification, Part II - Nonradiological Environmental Protection Plan, Section 5.4.2.

This documentation notification is made pursuant to Cook Nuclear Plant's Groundwater Permit GW 1810102, Part I.ll.b, Compliance Requirements.

The weekly (August 12 -

18, 2012) analysis requirement for Outfall OOE (Sanitary Sewage) was not completed for Ammonia,

Nitrite, Nitrate, Total Phosphorus and Biochemical Oxygen Demand (BODS).

The samples were obtained for the weekly requirement, but the analyses were not clearly communicated to the backup operator and subsequently, the laboratory analyst.

The regular operator was called away for a family emergency for three days, which caused the sample analyses to be missed by the back up operator.

This communication is to document the missed analyses, no effluent permit limitations were exceeded.

As required by Part I.ll.c, the schedule will require outfall OOE to be sampled for the parameters listed above during the week of August 19 -25 (due date August 25, 2012).

As required by Part I.ll.d, we will submit a written report to the Kalamazoo Office within 60 days (due date October 19, 2012).

The sanitary sewage Outfall OOE was resampled the following week, with all analyses completed.

The data for the parameters showed no permit exceedences.

Based on the data before and after the missed sample analyses, there is no adverse environmental impact from this event.

Evaluation of the cause for the analyses to be lost and the impact of that event to the groundwater:

The regular operator was called away for a family emergency for three days, which caused the sample analyses to be missed by the back up operator.

This communication is to document the missed analyses, no effluent permit limitations were exceeded.

Samples obtained for the previous week; and the week following the missed analyses showed no abnormal data.

There were no system upsets during the week of August 12 -

Proposal detailing steps taken to prevent recurrence:
