Dear Sir or Madam:

By letter dated September 29, 2010 (Reference 1), a revision to the final supplemental response to Generic Letter 2004-02 was provided. During the recent Arkansas Nuclear One, Unit 1 (ANO-1) refueling outage (1R23), insulation discrepancies were noted in which previously unidentified insulation was discovered. The purpose of this submittal is to inform the NRC of these discrepancies.

The following insulation discrepancies were noted during reactor building walkdowns in 1R23 and are being addressed under the corrective action program:

Metal encapsulated fibrous insulation (MEI) was discovered on top of the ANO-1 pressurizer. The MEI was not previously identified in Reference 1, Attachment 1, Revised Table 3.b.2-1 as a potential debris source; rather this pressurizer insulation was characterized as reflective metal insulation. An acceptable operability evaluation has been performed as part of the corrective action process. The MEI was determined to be outside of the 4D zone-of-influence based on MEI testing from another Entergy facility.

1CAN121101 Page 2 of 2 Temp-Mat fibrous insulation was discovered between the pressurizer and its restraint/support beams. An acceptable operability evaluation for the current operating cycle has been performed as part of the corrective action process. Plans for long-term resolution beyond the current operating cycle are being addressed under the corrective action program. Entergy will inform the NRC of the final plans for disposition by March 24, 2013.

Temp-Mat fibrous insulation was also discovered in metal enclosures around the eight support lugs on the pressurizer. This insulation was removed from the reactor building prior to start-up from 1R23.

This fibrous insulation was discovered as part of walkdowns to identify undocumented insulation based upon industry operating experience. Additional inspections are planned in the upcoming refueling outages for both of the ANO units to provide confidence that additional undocumented fibrous insulation is not installed. This action is being tracked in the corrective action system.
