As part of the routine blind performance drug testing program, a spiked sample certified to be
" positive" for amphetamine was screened " positive" for the presence of amphetamine by Arkansas Nuclear One (ANO) Fitness For Duty personnel and submitted to the SmithKline Beecham Clinical Laboratories on December 30,1994. The result of this test was released to ANO on January 2,1995, as " negative." Upon receipt of this test report, ANO sent a second aliquot of the same sample to SmithKline Beecham on January 4,1995. On January 11,1995, the laboratory reported that the second aliquot had been screened " positive" and confirmed
" positive" for amphetamine. Due to the discrepancy between test results from the same sample, an investigation was requested of SmithKline Beecham on January 27,1995. ANO j
sent letters to SmithKline Beecham on February 14,1995, and March 23,1995, requesting the investigation results. ANO received the report of the investigation dated March 28,1995, on March 31,1995.
This report states that the original aliquot was screened " positive" for amphetamine; however, due to an enor by a technician, it was not submitted for gas chromatography / mass spectrometry (GC/MS) and was erroneously reported as " negative."
In accordance with paragraph 2.8(e)(4) of Appendix "A" to 10CFR26, enclosed is the investigation report provided to ANO by SmithKline Beecham concerning the unsatisfactory performance test result.

Dear Dr. Hodges:
The above mentioned specimen was submitted for testing on 30 December 1994.
It was received into the laboratory on 31 December 1994.
The sample was aliquotted for testing on the Abbott TDX analyzer. It appears on the TDX printout as specimen number 9, with the result HI >=T. The result for that test was a positive result.
The technician errored and entered the result as a negative instead of the positive.
The result was released as a negative on 2 January 1995.
Receipt of your letter and the submission of this sample for GC/MS confirmation revealed a
positive amphetamine and methamphetamine.
This specimen result was released under the accession number 277149F.
The concentrations of amphetamine and methamphetamine determined were 1169 and 1196 ng/ml respectively.
Copies of the first TDX printout, and a copy of the MFR are enclosed.
The individuals involved have been counseled in this matter.
We sincerely regret this occurrence, and have informed all staff involved in the Abbott TDX methodology to pay closer attention to detail in this area, as this process is a manual one.
We respectively regret the negative entry on the first result.
I hope that this information resolves this to your satisfaction.
