D OTHER Specify in_ Abstract below or in 1.0 DESCRIPTION OF THE EVENT On 9/3/2018 at approximately 1045 hours0.0121 days
0.29 hours
0.00173 weeks
3.976225e-4 months
the 1 J emergency bus (EIIS System - EA) de-energized and
- the 1 J Emergency Diesel Generator (EOG) (EIIS System -
EK, Component - OG) auto-started as a result of a loss of th~ "A" Reserve Station Service Transformer (RSST) (EIIS System -
EA, Component - XFMR). The loss of the "A" RSST was due to a bird strike on the "C" Phase Lightning Arrestor which caused a fault on the "C" phase on the High Side (34.5kV) of the transformer. The auto start of the 1 J EOG is a valid System Actuation and is reportable in accordance with 10 CFR 50.73(a)(2)(iv)(A).
Also, as a result of this event, the Unit 1 "A" Charging pump (EIIS System -
CB, Component - P) auto-started due to logic associated with the loss of the 1 J emergency bus. The pump was secured shortly after the 1 J EOG was aligned as the sole power source for the 1 J emergency bus.
2.0 SIGNIFICANT SAFETY CONSEQUENCES AND IMPLICATIONS
No significant safety. consequences resulted from this. event because the 1J EOG responded as designed to power the 1 J emergency bus within 10 seconds of the loss of power. The 1 J emergency bus was aligned to an additional offsite power source at approximately 1140 hours0.0132 days
0.317 hours
0.00188 weeks
4.3377e-4 months
on 9/3/2018 and the 1J EOG was secured at 1144 hours0.0132 days
0.318 hours
0.00189 weeks
4.35292e-4 months
. The health and safety of the public were not affected by this event.
3.0 CAUSE
The direct cause of the loss of the "A" RSST is a bird strike on the "C" Phase Lightning Arrestor.
4.0 IMMEDIATE CORRECTIVE ACTION(Sl The lightning a~restor was damaged as a result of the bird strike and was replaced prior to return!ng the transformer to service.

~.O ADDITIONAL CORRECTIVE ACTIONS.
The "A" RSST is being replaced under a previously planned design change. As part of that design change, animal guards are being installed to mitigate future animal intrusion vulnerabilities.
6.0 ACTIONS TO PREVENT RECURRENCE The "B" and "C" RSSTs are scheduled to be replaced as part of previously planned design changes. These RSSTs will also have animal. guards installed to mitigate future animal intrusion vulnerabilities.
7.0 SIMILAR EVENTS A similar event related to an animal contact in the North Arina switchyard was reported in LER N1 -:2016-003-00.
