10 CFR Part 50, Appendix B, Criterion V, required, in part, that activities affecting quality be accomplished in accordance with instructions, procedures, and drawings appropriate to the circumstance. Procedure D63, Installation Guidelines for Threaded Fasteners, Section 1.0 stated that this procedure was to be employed by maintenance personnel when removing and installing threaded fasteners. Procedure D63, Step 7.3.7 required that bolts/studs be torqued to the proper stress value. Contrary to the above, on May 15, 2008, licensee personnel tightened safety related fasteners without employing the proper stress values included in Procedure D63. This violation was of very low safety significance because the failure to
torque the fasteners to the required value did not impact the operability of the specific components. The licensee initiated
CAP 1137836 to document this issue. Corrective actions included a review of the torquing practices to ensure that the fasteners are appropriately installed
