Event Description

During routine startup the auxiliary operator noted leakage from the weld on the inlet to the No. 3 charging pump relief valve.

Charging was transferred to the No.1 charging pump.

The rate of leakage was approximately 12 gpm, total leakage estimated at 100 i'

gallons.

This event was reported as Reportable Occurrence 50-29/76-05 in accordance with Technical Specification E.2.b.(2).

Cause Description: The apparent cause of the weld crack was that a full

'i 1/16" pullback gap was not allowed during installation of the pipe and vibration induced fatigue failure resulted. The affected section was replaced in kind and returned to service.

Due to the repetiveness of this incident (the third weld failure due to vibration induced fatigue this year), the plant has prepared a design change to install a pulsation.

dampener and sweeping elbows on the charging pump discharge..The sweeping elbows have been installed on No. 3 charging pump.

The plant is now awaiting the approval of Proposed Change No.137, " Installation of Pulsation Dampener on the No. Three Charging Pump Discharge Header", to complete this change.
