CONSENT AND HEARING WAIVER FORM Tennessee Valley Authority hereby agrees to comply with the terms and conditions of a Confirmatory Order arising out of an alternative dispute resolution process to be issued relating to enforcement action No, EA-1 2-021. The Confirmatory Order will be immediately effective upon its issuance. By signing below, Raymond A. Hruby, Jr., General Manager, Technical Services, Watts Bar Unit 2 Project, Nuclear Construction, Tennessee Valley Authority, consents to the issuance of the referenced Confirmatory Order, effective immediately upon its issuance and, by doing so, agrees to waive the right to request a hearing on all or any part of the Confirmatory Order.
