overexposure of a radiographer working on the North Slope of
Alaska.
Landauer reported that for the month of May, an individual's badge read 144
Rem.

The following information was received from the licensee via email:
On June 20, 2012, Acuren USA reported a possible radiation overexposure of 145R to an individual working for our [Acuren USA] company under NRC License # 42-32443-01. After interviewing all individuals involved, and conducting an internal investigation, we [licensee] believe that this dose was not received by the individual. In addition, the crew did not work with a gamma source but rather with x-ray tubes the entire month. The individual in question worked reading the scanner inside of a vehicle rather than outside utilizing the x-ray tube. He was never around the radiation producing equipment while it was energized. It is apparent that this dose was not received by this person. Therefore Acuren USA respectfully requests the NRC rescind Event Notification 48040. We apologize for the inconvenience and appreciate all your help with this matter.
