INTERIM EVAL OF DEVIATION PART-21 NOTIFICATION

-SENSE AND TRANSFER MODULE RESISTIVE SHORTThe following excerpted report was received via e-mail:"Sense & Transfer Module (part number 41-09-339904) was provided to PSEG under purchase order4500726553.

The customer identified an abnormal curve trace (resistive short) on the integrated circuit installed at location IC825 pin 6 on this printed circuit board. The customer performed an evaluation which includeddissection of the subject IC [integrated circuit]

however, this dissection provided inconclusive results as to thecause of the IC anomaly."At this time, UCI requires additional time to evaluate the results provided by PSEG and to work with themanufacturer in identifying the root cause. UCI currently considers this issue to be an isolated event as noprevious instances of this failure have been observed by UCI to date. PSEG has returned the item to UCI forevaluation and rework [received by UCI on 3/25/2015].

1OCFR21 Interim Letter, UCI Not Capable to Complete Part 21 Evaluation WithinTimelineThe purpose of this letter is to provide the NRC with a report in general conformity of therequirements of 10CFR Part 21.21. On January 30, 2015 United Controls International issued aReturn Material Authorization (RMA) request to Public Service Electric

& Gas (PSEG). TheRMA was issued to request the return of a Sense & Transfer Module of part number 41-09-339904 that was provided to PSEG under purchase order 4500726553.

The customer identified an abnormal curve trace (resistive short) on the integrated circuit installed at location IC825 pin6 on this printed circuit board. The customer performed an evaluation which included dissection of the subject IC, however, this dissection provided inconclusive results as to the cause of the ICanomaly.At this time, UCI requires additional time to evaluate the results provided by PSEG and to workwith the manufacturer in identifying the root cause. UCI currently considers this issue to be anisolated event as no previous instances of this failure have been observed by UCI to date. PSEGhas returned the item to UCI for evaluation and rework.UCI notified PSEG on January 30, 2015 that the subject Sense & Transfer Module needed to bereturned.

Required information as per 1OCFR Part 21.21(d)(4) follows:(i) Name and Address of the individual or individuals informing the Commission Luis SanchezVice President United Controls International 205 Scientific DriveNorcross, GA 30092Page 1 of 3 UNITED CONTROLS INTERNATIONAL (ii) Identification of thefacility, the activity, or the basic component supplied forsuch facility or such activity within the United States which fails to comply orcontains a defect.Thomas & Betts Power Solutions/Cyberex Sense & Transfer Module of partnumber 41-09-339904 was found to have abnormal curve traces at pin 6 of theintegrated circuit installed at board location IC825. The item was supplied toPublic Service Electric

& Gas under Purchase Order 4500726553.

(iii) Identification of the frm constructing the facility or supplying the basiccomponent which fails to comply or contains a defectUnited Controls International 205 Scientific DriveNorcross, GA 30092(iv) Nature of the defect or failure to comply and the safety hazard which is createdor could be created by such defect or failure to comply.UCI requested the return of the subject Sense & Transfer Module from PSEG onJanuary 30, 2015. UCI received the subject item on March 25, 2015 and thus, hasnot had adequate time to coordinate efforts in evaluating the subject item with themanufacturer and customer.

(v) The date on which the information of such defect or failure to comply wasobtained.

UCI shall determine whether the component installed at board location IC825 is indeed the root causeof failure.

The manufacturer has been requested to provide the soonest available date for testing and is expected to be performed within the next two months.(viii) Any Advice related to the defect or failure to comply about the facility,

activity, or basic component that has been, Is being, or will be given to purchasers orlicensees.
