Theplungeroftheshunttripassemblywasbindingintheshunttripcoil.Whenactivated, thebreaker's shunttripcoilplungerpullsthebreakertripperbartotripthebreaker.Theboundplungerwasnotreturning toitsdeenergized position.

Thebreakertripperbarwasbeingmaintained inthetripfreeposition, preventing theDB-75aircircuitbreakerfromclosing.

Page3Testingofthe"B"EDGSupplyBreakertoBus16wasperformed inNovember, 1997,duringthe1997outage.'Duringthistesting,thebreakerfailedtoclose.Thebreakercouldnotbeclosedelectrically ormanually, becausetheshunttripcoilplungerwasmaintaining thetripperbarinthetripfreeposition.

Thisfailurewouldpreventthe"B"EDGfromsupplying powertothesafeguards loadsonBus16.Thisisconsidered amajordegradation since,inconjunction withasinglefailureontheredundant train,requiredsafetyfunctions couldnotbeperformed.

THEDATEONWHICHTHEINFORMATION OFSUCHDEFECTORFAILURETOCOMPLYWASOBTAINED:

Theinformation wasobtainedduringtestingperformed onNovember14,1997.INTHECASEOFABASICCOMPONENT WHICHCONTAINSADEFECTORFAILSTOCOMPLY,THENUMBERANDLOCATIONOFALLSUCHCOMPONENTS INUSEAT,SUPPLIEDFOR,ORBEINGSUPPLIEDFORGINNASTATION:TherearesixWestinghouse DB-75breakerspermanently installed atGinnaStation,insafeguards buses14,and16.Oneofthesebreakersismanuallyoperatedandtheshunttripcoilisnotused.Inaddition, thereisonespareDB-75breakerinstock.Withtheexception ofthe52/EG1B1breaker,thesebreakersstillhavetheiroriginalshunttripassemblies installed.

The52/EG181breakeristheonlyDB-75breakerthathashadtheshunttripassemblyreplaced.

Thereplacement shunttripassemblythatfailedwasoneoftenpurchased in1977asspares.THECORRECTIVE ACTIONWHICHHASBEEN,ISBEING,ORWILLBETAKEN;THENAMEOFTHEINDIVIDUAL ORORGANIZATION RESPONSIBLE FORTHEACTION;ANDTHELENGTHOFTIMETHATHASBEENORWILLBETAKENTOCOMPLETETHEACTION:Corrective actioniscomplete.

Thefailedshunttripassemblywasreplacedwithasparefromstock.Theassemblywasmanuallyandelectrically bench'tested priortobeingreturnedtooperablestatus.Theremaining eightassemblies instockwereinspected.

Twoassemblies hadcoilplungersthatdidnotfullyreturntothedeenergized positionduringmanualmanipulation.

Thesewereremovedfromstock.1TheotherDB-75breakerswerevisuallyinspected.

Acceptable plungermovementisindicated byadequateplungertotripperbargapandfullretraction ofthealarmswitchattachment pushrod.Noadditional bindingconcernswerefound.

Priortoreordering, thePAFnowrequirestheadditionofplungermovementverification totheassemblyacceptance plan.Westinghouse wascontacted.

Therehavebeennootherreportedeventsofthisnature'that Westinghouse isawareof.Acheckofindustryoperational eventsdidnotidentifysimilarbreakerfailure.ANYADVICERELATEDTOTHEDEFECTORFAILURETOCOMPLYABOUTTHEFACILITY,

Receiptinspection couldverifyacceptable plungermovement.
