Technical Specification 3.7.6.1 states
control room emergency ventilation system requires two independent
control room emergency ventilation systems (monitors) be
operable while plant is operating in modes 1, 2, 3, and 4. With both channels of station vent normal range radiation monitoring instrumentation
inoperable, TS
LCO 3.7.6.1 Action C requires that within one hour the control room normal ventilation system be isolated and at least one
control room emergency ventilation train placed in operation. Contrary to this requirement, on October 16 and 22, 2007, both trains of Station Vent Radiation Monitors were
inoperable for more that one hour without entering into
TS 3.7.6.1 Action C. This issue was entered into the licensees Corrective Action Program as CR 07-29410. The issue is of very low safety significance because it did not involve
ALARA planning or work controls, an
overexposure,
substantial potential for overexposure, or the ability to assess radiation dose
