CLOSURE OF INVESTIGATION (OI 4-2024-003)
Dear Fadi Diya:
This refers to an investigation conducted by the U.S. Nuclear Regulatory Commissions (NRCs)
Office of Investigations regarding activities at Callaway. The Office of Investigations initiated the investigation to determine whether licensee personnel willfully ordered maintenance personnel to strike a check valve over the course of multiple shifts despite having knowledge the practice was not acceptable and the vendor recommendations were not being followed.
Based on the evidence developed during the investigation, the concern that licensee personnel willfully ordered maintenance personnel to strike a check valve over the course of multiple shifts despite having knowledge the practice was not acceptable and the vendor recommendations were not being followed, was not substantiated. The NRC plans no further action related to the alleged wrongdoing and considers this investigation closed.
