While running SI 4.5.E.1.d and e on the HPCI, PCV-73-43 f ailed to operate properly.

Conditions Prior to Occurrence:

Unit 1 - 93% steady state power.

Unit 2 - 96% steady state power.

Unit 3 - cold shutdown Action specified in the Technical Specification Surveillance Requirements met due to inoperable equipment.

Describe.

CS, DIR, RCIC, and ADS logic proven operable.

A_pparent Cause of Occurrence:

The valve was stuck closed due to the main valve guide becoming loose and unthreading.

Analysis of Occurrence:

There was no danger to. the health or safety of the public, no significant resulting chain of events, no release of activity, and no damage to plant or equipment.

Corrective Action

The valve was disassembled aad cleaned and the valve guide tightened. No recurrence control is required due to no previous failure history.
