Dear Sirs:

Fursuant to the requirements of Section 5.7.B of the Technical Specifications submitted as Proposed Change Ho. 105, we hereby report the following abnomal occurrence, designated A.O. No. 73-1.

On April 2, 1973 at 0815 hours0.00943 days <br />0.226 hours <br />0.00135 weeks <br />3.101075e-4 months <br /> a contractor employee opened the vanor container personnel hatch outer door with the inner door alreadvD

The main coolant pressure and temperature were 680 psi and h00 F, open.

respectively. The main coola-t baron concentration was that required for 75 cold shutdown. The outer door remained open for approximately two minutes.

Sinultaneous with the openitv; of the outer door there was an A calculated volume of 12,560 inadvertent release of tritin as a vapor.

of air containing a total activity of 82.3 uc of tritium was released ft3 frcm the vapor container. une concentration when averaged over a 2h hour period was 3.3 x 10-10 uc/ce, vell below the MPCa of 2 x 10-7 uc/cc.

A breach of vapor container integrity when the main coolant sycten is above 200 F and abcve 300 psig with the reactor core in place constitutes a violation of our Technical Specifications.

The entire Contractor work force was indoctrinated en the re-quirenents fcr vapor container integrity and instructed that entry during intecrity would require plant personnel accc paniment.

The vapor container personnel hatch door controls were posted vlth written instructions for their proper cperation.

The procedure for entr/ into the vapor container while maintain-inc the integrity of same vill be incorporated into the Health Physics Indoctrinaticn Program for contractor personnel.
