Department of the Navy
' License No. 04-06145-01 Long Beach Naval Shipyard Long Beach, California 90822 As a result of the inspection conducted on March 24, 1981, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7,1980), the following violations were identified:
A.
10 CFR 34.43(c) requires that a survey record shall be maintained for two years for the last survey conducted prior to locking the radiographic exposure device and ending direct surveillance of the operation.
Contrary to the above requirement, a record of the final survey was not completed for the following radiographic operations:
1.
January 7,1981 at the Naval Ship Center, Job Order 81056-10189.
2.
January 23, 1981 on the "PELEJU", Job Order 14242-25291648.
The above items constitute a Severity Level VI Violation.

B.
License Condition 15 states that the licensee shall possess and use licensed material in accordance with statements, representations, and procedures contained in application dated June 2,1978.
Chapter 1 of the " Administrative, Operating, and Emergency Procedures for Industrial Radiography and Radiac Calibration Programs" (hereafter referred to as Reference 1) was submitted as a part of the application.

Chapter 1, Reference 1, requires that radiographers and radiographer's assistants who have satisfactorily completed i
the training requirements for these positions to be certified as such by the shipyard.
l (1) Contrary to the above requirement, J. F. Rivas signed the radiographic worksheet on February 27, 1981 as a radiographer for the work done on the "J. P. JONES" using an iridium-192 l
source having the serial number 14187.
An examination of the records indicated that Mr. Rivas had not been certified as a radiographer by the shipyard.
l (2) Contrary to the above requirement, Ransom Anderson was I
also acting as a radiographer in the shipyard; however, i
there was no dncumentation to indicate that he had been properly certified by the shipyard.

Chapter 1, of Reference 1, requires visual inspections of the AN/UDM-1A radiac calibration source to be conducted on a monthly basis.
Contrary to the above requirement, an examination of the inspection records for the AN/UDM-1A radiac calibration source indicated that a visual inspection was not conducted during the month of September 1980.
3.
Chapter 1, of Reference 1, requires audits of radiac calibration mechanics and radiac operations to be conducted on a quarterly basis.
Contrary to the above requirement, at the time of the inspection, the records indicated that only one audit had been conducted on October 6,1980 of radiac calibration mechanics and radiac operations for the year 1980.
4.
Chapter 1, of. Reference 1, requires quarterly radiation surveys to be made of the radiographic facility, Building 52.
Contrary to the above requirement, an examination of the survey records, at the time of the inspection, indicated that radiation surveys of the radiographic facility, Building 52, were conducted only on January 11, 1980, April 8, 1980, and August 14, 1980.
The above items constitute a Severity Level V Violation.
(Supplement VII)
Pursuant to the provisions of 10 CFR 2.201, Long Beach Naval Shipyard is hereby required to submit to this office within twenty-five days of the date of this Notice, a written statement or explanation in reply, including:
(1) the corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further items of noncompliance; and (3) the da+' when full compliance will be achieved.
Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or affirmation.
