"* NJ Docket No. 99900724/80-01 NOTICE OF DEVIATION Based on the results of an NRC inspection conducted October 20-21, 1980, it i
appears that certain of your activities were not conducted in accordance with NRC requirements as indicated below:
Criterion V of Appendix B to 10 CFR Part 50 states in part; " Activities affecting quality shall be prescribed by documented instructions, pro-cedures, or drawings of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures or drawings...."
Paragraph 5.B. of IEW Ultrasonic Weld Irspection Procedure U.T. 122, Revision 0, states in part, "The beam angle in the production material shall be in the range of 40 to 75 degrees inclusive with respect to the perpendicular to the entry surface," and "9.C... the search unit shall be placed on the contact surface with the beam aimed at about 90 degrees to the weld and manipulated laterally and longitudinally so the ultrasonic beam passes through all of the weld in two different approaches of the beam to the reflector."
P Contrary to the above, the IEW technician ultrasonic examined the "T" welds using a forty-five (45) degree shear wave which failed to achieve the requirement of the ultrasonic beam passing through all of the weld in two different approaches of the beam to the reflector.
The inspector verified that all corrective action had been completed on the five (5) contracts with ES for the supports, stops, and restraints supplied to LPL Waterford, SES, Unit 3.
No response to this deviation is required.
