DISCUSS HRC STAFF CONCERNS RELATDG TO TWO RECENT FAILURES OF DIESEL GENERATOR I

l On July 31, 1981, the NRC staff requested a meeting with management repre-t sentatives of APCo at Bethesda, Maryland. A list of attendees is attached.

The purpose of the meeting was to discuss NRC staff concerns mlating to diesel generator (DG) 14, failures. The failuras resulted in issuance of l

emergency license amendments on May 8,1981 and on July 31, 1981. The first amendment authorized a 10-day extension in the allowable outage time (72 hours8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br />) and the second amendment authorized a 6-day extension. Both amendments related to DG 1C which tustained similar type failures (water l

in cylinder) during attempts to ;,erfonn surveillance testing. The sur-l veillance testing was specifically required by Technical Specifications l

and Regulatory Guide (RG) 1.108 which was implemented during licensing of l

Unit W. 2.

The APCo Vice President of Nuclear Generation, Mr. R. P. Mcdonald, reviewed l

the suuation effecting riiesel generator 10. The diesel is one of two l

Fairbanks Horse type 38D8-1/8 series OP, 2850 KW, 900 RPM used,at Farley l

site. Forty-eight similar machines are reported to be in service at nuclear j

facilities. The Farley site uses three additional 4050 KW diesel generators j.

in addition to the two 2850 KW diasels for station emerrency power. DG 1C has about 400 operating hours. DG 1C and DG 2C (similar for Unit 2) have had about 150 starts for surveillance testing per RG 1.108 since March,1981.

On May 8, i981 Nater was funnd in cylinder #10 of DG 10. A cylinder o-ring had failed resulting in cooling water entering the cylinder. The DG 1C had i

been tested in late April,1981 to raquirements of the Technical Specification and RG 1.108. Part of the testing was a 24-hour load run including 10% over-load for two hours. APCo reported that due to operator error during tests on DG IC it exceeded the-10% overload value of 2838 KW on two noted occassions to loads of 3500 KW (136%) and 3300 kW (128%), respectively for short periods.

connecting rod wrist pin bushing was damaged causing cylinder ovedienting.'

t and other damages. Repairs included mplacement of twelve cylinder-liners l

(new design with two seals vice one), the vertical drive assembly, and piston rings, etc. APCo stated thattthey now considered DG 1C to be in a completely overhauled condition. -Cylinders 31, #3, #5, #9. and #12 had shown leaking into the cylinders during hydro-tests.

Cadse of the failums appear to be the over.lca_d tests which; caused the upper wrist-pin bushing failures. Consequently, the piston appeared to have over-heated the e,ylinder sleeves resulting in o-ring failures. APCo and the Colt Industries (DG vendor) representative (Mr. J. Tangye) consider that checking the measurements of the wrist-pin budhing extended length may indicate poten-tail-for failures of this type. Therefore, DG 2C, the other DG of this cesign -

at Farley Plant will be checked in this manner.

A discussion ensued ralating to the testing to be done prior to accepting DG 1C for service at Farley Site. APCo and the DG representative will provide them to NRC in their follow-up mport on the DG failums.

In addition, the DG vendordrepresentative, Mr. J. Tangye, stated that he had not previously corre mented on RG 1.108, but the testing per RG 1.108 would not be his mcomendation.

1 He stated that the 72-hour outage time requirement appeamd overly mstrictive and may result in short-cuts in getting repairs done in a thorough manner. He advised that the overload testing probably damaged DG 1C at FarHy. As to out-age time, he agreed that most minor repatts could easily be done within the existing 72-hour allowable outage time.

In conclusion, APCo-agreed to pmvide the following information to the NRC staff?

(1) Details o'f the DG 1C failure and repairs accomplished, (2) Details of testing accomplished on DG 1C to assure its operability, and t

(3) The recent mport issued by a special APCo DG Task Group which includes recomendations for improvements to the 0G mliability at Farley Plant.

Iteras (1) and (3) were included in the NRC Staff Safety Evaluation Report for License Amendments 21 and 3 for Farley Units 1 and 2 on July 31,1981. APCd had agreed to provide these reports.
