Description of Occurrence: A critical approach was under way on May 3,1974 when the shift supervisor on duty realized that Check
- : i Sheets C-2 and C-3 had not been completed just prior to the critical 1
approach. All rods were then inserted and the check sheets completed l-before continuing with the critical approach.

Designation of Apparent Cause of Occurrence: Procedural deficiency

Analysis of Occurrence: Required checks for the entire refueling outage vere being checked off on a single master checklist. Tra-ditionally, more than one criticality ir completed during refueling outages and Check Sheets C-?: neutmn monito; ing out of core, and

C-3, reactor protection system,.had been properly completed on

April 13 and April 16 prior to loadica fuel and C-2 had also been 1
done on April 20 prior to the head-off critical. The r. aster check-list showed these tests complete for April 16. It was felt that they need not be run again. This is contrary to the requirements 3
of Technical Specifications Section 7 3 2 which implies that they shall be done for n cold start.up after an extended shutdown. The l
plant was asked by AEC/DRO last Dece=ber 1973 to define extended f
shutdown and it was at that time defined as a 30-day shutdown.

Based on a review of this incident on May 3,197h by the Plant l
Review Co::nittee, an extended shutdown vill nov be defined as any 8
cold shutdown and requirements of co=pleting Check Sheets C-2 and i
C-3 vill be met just prior to start-up from cold shutdown and completed within 48 hours5.555556e-4 days <br />0.0133 hours <br />7.936508e-5 weeks <br />1.8264e-5 months <br /> prior to start of the critical approach.
Traditionally, Caeck Sheets C-2 and C-3 have been run prior to hot i
l start-ups, as well as cold start-ups, and recent procedure and checklist revision work did not factor in these long standing require ents. Since the more co=prehensive reactor protection l
system checks by the inattument and control group had been ecm-pleted during the outage and all neutron monitoring system repair

/0077 work had been properly completed and verified operational prior to this start-up, it is felt that no undue hazard existed because of this misinterpretation.
9 Corrective Action: Check Sheets C-2 and C-3 were completed prior to continuation of physics testing. The master checklist was revised to include requirements of completing C-2 and C-3 check sheets as outlined in Paragraph 8 10.
