This refers to the special inspection conducted by Mr. W. B. Jones and

G. E. Werner of this office on December 14, 1992, through January 21, 1993.

The inspection included a review of activities authorized for the Comanche

~

Peak Steam Electric . Station, Units 1 and 2, facility. At the conclusion of

,

the inspection,.the findings were discussed with you'and those members of your

staff identified in the enclosed report.

This special inspection was conducted to review programmatic and

-

implementation concerns with the station temporary modification program.

In

,'

addition, the effectiveness of previous corrective actions to correct

repetitive deficiencies with this program were evaluated.

i

Specific areas examined during the inspection are identified in the report.

l

'

Within these areas the inspection consisted of selective examinations of

procedures and representative records, interviews with personnel, and

!

observatiog of activities in progress.

'

Based on the results of this special inspection, certain licensed activities

l

-

. appeared to be in violation of NRC requirements as 'specified in the-enclosed

!

Notice of : Violation (Notice). . The violations are significant in that

!

repetitive problems with the implementation of the station temporary -

' modification program were identified but prompt- and effective corrective

}

.

actions were not taken to correct the deficiencies. -These deficiencies

included both technical- and administrative implementation problems. Numerous

occurrences were noted-where vital station drawings, used by operators to .

,

assess clearances and work-activities, were.not marked up to show active

.

temporary modifications. .Two significant incidents were noted where temporary-

.

'

modifications were not _ reviewed for their impact. on pending clearances.

The January 15, 1993, management meeting on the temporary modification program

.

-provided a comprehensive. overview of-the temporary modification program

deficiencies discussed in this report.

During the presentation, your staff

identified several interim corrective actions which were being taken to

address the implementation concerns. The NRC subsequently reviewed the

effectiveness of the interim corrective actions and verified that the

temporary modification program was being adequately implemented.

You are required to respond to this letter and should follow the instructions

specified in the enclosed Notice when preparing your response.

In your

response, you should document the specific actions taken and any additional

actions you plan to prevent recurrence.

We request that your response include

,

an assessment of open plant incident reports greater than 90-days old. After

reviewing your response to this Notice, including your proposed corrective

actions and the results of future inspections, the NRC will determine whether

further NRC enforcement action is necessary to ensure compliance with NRC

regulatory requirements.

In accordance with 10 CFR Part 2.790 of the NRC's " Rules of Practice," a copy

of this letter and its enclosures will be placed in the NRC's Public Document

Room,

The responses directed by this letter and the enclosed Notice are not subject

to the clearance procedures of the Office of Management and Budget, as

required by the Paperwork Reduction Act of 1980, Pub. L. No. 96.511.
