APPENDIX-A Anchor Darling Valve Company Docket No. 99900053/81.

NOTICE OF NONCONFORMANCE Based on the results of an NRC inspection conducted on October 26-30, MSi. it appears that certain of your activities were not conducted in ac'cordance with NRC requirements as indicated below:

Criterion V of Appendix B to 10 CFR Part 50 states:

" Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, prccedures, or drawings.

Instructions, procedures, or drawings shall include appropriate quantitative or qualitative acceptance criteria for determining that important activities have been satisfactorily accomplished."

Anchor Darling Valve Company (ADVC) Drawing ho. 9362F for the 30 x 24 x 30 inch double disc, parallel seated gate valve body, requires a by pass on the right side of the valve body when facing the valve inlet.

Contrary to the above, two gate valves on Order No. 1024-001-001 for Washington Public Power Supply System Nuclear Project 2 were shipped with i

the by pass installed on the opposite side of the valves to that specified by Drawing No. 9362F.

Paragraph 4.2.1.10 in Westinghouse Equipment Specification 675198 states, "All valve assemblies shall be designed to withstand seismic loading equivalent to 0.76g in the horizontal direction and 0.51g in the vertical direction.

Under these conditions the sum of the primary, secondary, and seismic stress shall be limited to 0.9 times the yfeld stress,"

i Contrary to the above, it could not be arified that the yoke mounting flange in San Onofre Unit 1,14 x 10 x 14 inch gate valves was designed to withstand the specified seismic loading and applied stresses, in that an incorrect equation was used for performing the analysis, i.e., an equation for a circular plate with inner edge fixed and supported and uniform loading along the outer edge (Roark's " Formula for Stress and l

Strain," 4th Edition, Case 22) was improperly used for analysis of a flange consisting of two abutting semi-circular plates.
