HIGHLIGHTS OF WEEKLY.INFORMATION REPORT WEEK ENDING SEPTEMBER 28, 1990 Meeting with 00E on Substandard Vendor Products At DOE's request the Director. 01 and the Chief, Vendor Inspection Branch are meeting on September 27, 1990, with DOE staff to discuss NRC activities over the past few years to address substandard vendor product issues. The focus of the meeting will be on 01 and VIB actions to review instances of suspected counterfeit or fraudulent marketing by venders, suppliers, and distributors to nuclear power plants. Copies of previously issued information notices and bulletins will be provided to 00E. B&W Fuel Company (B&W), Lynchburg, Virginia On September 24, 1990, Saecial Nuclear Material 1.icense No. SNM-1168 for the Commercial Nuclear Fuel )lant was renewed for a 10-year term. Th'.s action followed preparation of an Environmental Assessment in May 1990 and publication 24, 1990. of a " Finding of No Significant Impact" in the Federal Register on May The license authorizes B&W to fabricate enriched uranium oxide pellet:s into fuel elements for light water reactors. This is the first 10-year renewal following Commission approval of 10-year licenses for major operating fuel cycle licenses (SECY-89-364), publication of a Notice in the Federal Register on June 19, 1990, and Office of Management and Budget (OMB) clearance for an update of the safety section every 2 years. Earthquake in the New Madrid Area A small earthquake of magnitude 4.6 occurred in the New Madrid area of south-eastern Missouri at 7:19 a.m. on September 26, 1990. The epicenter was located about 10 miles southwest of Cape Girardeau and 40 miles north of New Madrid at 37.2'N, 89.7'W. The earthquake was felt in parts of Missouri, Illinois, Kentucky, Ohio and Arkansas; and it caused minor damage in southern Illinois. The nearest nuclear power plant is Callaway located in central Missouri at a distance of 150 miles from the epicenter. The earthquake was too small to cause'any operational problems at the plant. Observer teams from several NRC-supported universities and the USGS have been dispatched to the area to record aftershocks. Memphis Sta+e University recorded a magnitude 2.8 af tershock at 8:47 p.m. on September 26. This aftershock was felt by the crew in the field. A DOE facility in Paducah, Kentucky, observed a ground motion of between.05 and.10 g-ie a.05g instrument triggered and a.10g instrument did not. USA Today Hotline TheDivisionofContractsandPropertyManagement(DCPM)hasagreedto represent the NRC on October 3, 1990, on a hotline set up by USA Today for small businesses to discuss. procurement concerns with Federal agencies. DCPM was asked by the Office of Small and Disadvantaged Business Utilization / Civil Rights, to participate in this activity. Second Building Status 1 On September 25, the Office of Consolidation (CONS) staff met with Dewberry & Davis space planners and Tower Construction's managing architect to discuss Tower's proposed proto-typical floor plan for Two White Flint North (TWFN) office space. The typical plan for a floor in TWFN which is all. office space will accommodate just over 200 people, or about twice as many as the most densely populated floor in One White Flint North (0WFN). However, there will be greater variation in layout and densities in TWFN than in OWFN because of the greater number of special and support spaces to be incorporated. 1

a Office of Nuclear Reactor Regulation-Items of Interest Week Ending September 28, 1990 H. B. Robinson Effective September 24, 1990 Richard E. Morgan was reassigned from Plant General Manager to Assistant to the Project Manager (Charles Dietz). J. J. Sheppard, formerly the Operations Manager, assumed the position.of Plant General Manager. The Manager of Operations is now R. H. Chambers. Yankee Rowe Yankee Rowe had gone from Mode 4 (hot shutdown) to Mode 3 (hot standby) on September 26, 1990. A steam leak was observed at an Instrumentation penetration through the head of the reactor vessel. The leak has been approximated at lec of water per minute. The temperature was approximately 400' and the pressure was approximately 600 psi. A Unusual Event was declared at approximately 9:00.a.m. on September 27, 1990. The licensee expects to be down for approximately 0 days while the leak is repaired. e e Office of Nuclear Material Safety and Safeguards items of Interest Week Ending September 28, 1990 i Licensee Meeting on International Safeguards On September 25, 1990, representatives of the Westinghouse i.ow Enriched Uranium fuel Fabrication Plant in Columbia, South Carolina met with International Safe-guards Branch to discuss the status of the facility with respect.to current and future international safeguards requirements. Effective in January 1988, the f acility was removed from the list of selected facilities for the application of safeguards pursuant to the Safeguards Agreement between the United States and the International Atomic Energy Agency. Concurrently, the facility was selected for reporting of nuclear material transaction and inventory data under the terms of the protocol to the referenced agreement. Scheduling of future physical inventories, potential timing of future reselection pursuant to the Agreement, and the possible impact on future inspections of new fuel designs and throughput-dominated operating philosophy were discussed. Meeting on BR-100 Shipping Cask On September 19, 1990, the Transportation Branch staff met with Babcock and Wilcox (B&W) and the Department of Energy (DOE) to. discuss progress on the BR-100 spent fuel shipping cask. The BR-100 cask is one of a series of spent fuel shipping casks being designed for shipment of spent fuel under the Nuclear Waste Polic The 100-ton rail cask has a capacity of 52 boiling water reactor (BWR)y Act. assemblies or 21 pressurized water reactor (PWR) assemblies. B&W plans to conduct a series of quarter scale tests to evaluate'the structural performance of the cask and impact limiters. The tests will be conducted in the Summer of 1991 at Oak Ridge National Laboratory. B&W expects to submit an application for the cask in January 1992. B&W Fuel Company (B&W), Lynchburg, Virginia On September 24, 1990, Special Nuclear Material License No. SNM-1168 for'the Commercial Nuclear Fuel Plant was renewed for a 10-year term. This action followed preparation of an Environmental Assessment in May 1990 and. publication of a " Finding of No Significant Impact" in the Fedetal Register on May 24, 1990. The license authorizes B&W to fabricate enriched urtiiTum oxide pellets into fuel elements for light water reactors. Low-Level Waste Facilities in Michigan and New Jersey The Division' of Low-Level Waste Management and Decommissioning recently received documents related to the development of low-level waste facilities from.two non-- Agreement States, Michigan and New Jersey. -2 l ~ Waste Authority has provided the staff, for review and comment, reports related to site characterization, man-made features, statutory and regulatory compliance, and quality assurance. The New Jersey Low-Level Radioactive Waste Disposal' t Facility Siting Board provided, through GPA, its proposed methodology for site screening and identification and a request for proposal for a site character-izatior, contractor. Staff anticipates completing a broad scope review of these documents by the end of October. Performance Assessment Workshop A low-level Waste Management Performance Assessment Workshop on computer modeling was held for 16 Agreement State regulators at NRC's Information Technology on September 26-28, 1990. This workshop was sponsored by the Office of Governmental and Public Affairs, State Pronrams, with technical support provided by the Division of Low-Level Waste Aanagement and Decommissioning and Sandia National Laboratory staff. l ~ Office of Nuclear Regulatory Research ltems of Interest Veek Ending September 28, 1990 i International Workshop on Nuclear Plant Safety and Life Extension Programs On September 19 and 20 the Deputy Director, Division of Engineering attended an International Workshop on Nuclear Plant Safety and Life Extension Programs in Newport, Rhode Island and presented a paper on " Engineering Technical i Issues in Plant Safety and Life Extension from a Regulatory Point of View". l The workshop was jointly developed by ANS and ASME in response to a request i from Academician K. V. Frolov, VP of the USSR Academy of Sciences and member of the USSR Supreme Soviet. A twelve person Soviet delegation led by Academician Frolov attended the conference. Gormley 492-3743 Meeting on South Texas Fire PRA Representatives of NRR, RES/PRAB and Sandia National Laboratories met with representatives of Houston Power and Light Company on September 19 and 20, 1990, to discuss the fire analysis in the South Texas PRA. Unlike the usual SEPTEMBER 28,'1990 ENCLOSURE C ij

-2 experience with other plants' PRAs, where fire-initiated sequences are quite significant in the plants' total safety profile, the fire-initiated sequences in the South Texas PRA are a relatively minor contributor to the total core damage frequency. This somewhat unusual finding appears to be well explained i by the high (3-train) redundancy of safety systems in this plant, along with a greater than normal amount of physical separation between the various trains. g Participation in the FARO-TERMOS Procram The Division of Systems Research, Office of Nuclear Regulatory (Research, is now ready to participate with the Safety Technology Institute STI)ofthe Commission of the European Communities Joint Resecrth Center (JRC), ispra, Italy, on severe accident research related to molten fuel-coolant interactions. Under the FARO-TERMOS Program, ST1-JRC will perform severe fuel damage experiments to provide experimental data on the rate of the hydrogen and steam produced, fuel fragmentation, and the thermal and mechanical loads imposed on the lower plenum structures and the lower head of the reactor pressure vessel as a result of the molten-fuel-water-structure interactions. This is a three year program expected to cost $1.3 million. If the NRC had to build similar facilities, the cost would be in excess of the $12 million which the JRC already spent on the facility.- The NRC contribution of $1.3 million will be used to perform some minor facility modifications and to' defray a small fraction of the operating cost while giving us access to the test data. This Technical Exchange Arrangement covered a three-year period from September 1990 to August 1993, at a cost of $1.3M to the USNRC. i SEPTEMBER 28, 1990 ENCLOSURE C

i Office for Analysis and Evaluation of Operational Date Items of Interest Week Ending September 28, 1990 i Incident Response On September 26 and 27, IRB staff presented a course on Technical Procedures for Emergency Response for states and utilities in the Region I area. The course e was sponsored by the Office of State. Programs and hosted by the State of Maryland. t ULYSSES SPACE LAUNCH: A German built, European S> ace Agency sponsored I nuclear-powered spacecraft known as Ulysses is scleduled to be launched from-Kennedy Space Center sometime during the period of October 5 to October 21, 1990. NASA prepared procedures for a Federal response in support of the States in the i event of a radiological emergency during the launch. The_ Federal Government's response will be in accordance with the Federal Radiological Emergency Response Plan (FRERP), Major portions of the FRERP.will be activated before the launch to be p epared in case of an emergency. NASA will be the Cognizant Federal 1 Agency CFA) for the Ulysses operation and will operate from its Radiological i Control Center. AFederalRadiologicalMonitoringandAssessmentCenter(FRMAC) l and a Joint Information Center (J1C) will also be established near the Kennedy I Space Center. FEMA will establish a Federal Response Center-(FRC) in case of an actual emergency. When the NRC is not the Cognizant Federal Agency, as in this case, its role is minor. Federal agencies that may have a major role in-a FRERP response will be deploying to the site prior to launch. Since NRC does not have a significant role, our response would occur only in the case of an actual emergency. \\ j Office of Governmental and Public Affairs Items of Interest Week Ending September 28, 1990 Transportation Course Kathleen Schneider, State Programs, conducted a nuclear transportation course for State regulatory personnel on September 24-28, 1990 in Houston, Texas. The course had twenty-one students and included instructors from the NRC, State governments and industry. The course covered a ~ variety of transportation-- related items such as inspection and enforcement, transportation of-low-level i waste, radiopharmaceuticals, radiography and well-logging equipment, and emergency response to a transportation accident. Industrial Radiography Course 4 James Myers, State Programs, conducted a course on the safety aspects of industrial radiography on September 24-28, 1990 at the Amersham Corporation in Burlington, Massachusetts. Thirteen State inspectors participated in the. training course. The course covered various aspects of industrial radiography safety and included a tour of a radiography camera manufacturer, a visit to an operational radiography site and an inspection of a radiography company. Entrance Meeting for New York Radiation Control Programs Review Carlton Kammerer, Director, State Programs,-Richard Cooper, Deputy Director, m Region I Division of Radiation Safety and Safeguards, and John McGrath. Region 1 State Agreements Officer, met with senior New York State and City officials from the State Energy Office, Department of Environmental Conservation, Department of Labor, State Health Department and the New York City Department of Health on September 26, 1990 in Albany, New York. The purpose of the meeting was to discuss the consolidated review of all the New York radiation control programs scheduled for October. Protective Measures Workshop State Programs sponsored a Protective Measures Workshop.on September 26-27, 1990 at the O'Coner State Office Building in Baltimore, Maryland. The workshop wds Conducted by representatives from the Office for Analysis and Evaluation of Operational Data and included approximately 30 emergency preparedness and radiological health personnel from States in Region I and Virgini.a. Utility emergency preparedness officials also participated in the workshop. This was a pilot workshop which State Programs. intends to sponsor-in other Regions.in the i future. A Notice of Violation and Proposed Imposition of Civil Penalty in the amount of $7,500 will be issued on.or about September 24, 1990 to Tri-State Associates, Inc. This action is based on a whole body exposure to a radiographer in excess of the quarterly limit, failure to evaluate and report the overexposure, and failure to submit required reports to-i the Commissior.. The civil penalty was escalated for identification and reporting and past performance and mitigated for corrective action, with a net escalation of 50%.- (EN 90-087) 2. A Notice of Violation and Proposed Imposition of Civil Penalty in the-amount of $75,000 will be issued on or about September 26, 1990 to Arizona Public Service Company. This action is based on the licensee's failure to have an adequate program to ensure that all-licensed operators are medically qualified as required by 10 CFR Part 55. (EN 90-088) Civil Penalty Paid 1. Consolidated NDE, Inc., Woodbridge, New Jersey, paid the civil penalty in the amount of $10,000. The action was based on a number of violations-that represented a significant lack of attention toward a. system of NRC requirements intended to protect against exposure in excess of regulation limits. ) i l Office of Consolidation Items of Interest Week Ending September 28, 1990 Second Building Status i I On September 25, the Office of Consolidation (CONS) staff met with Dewberry & Davis space planners and Tower Conttruction's managing architect to discuss Tower's proposed proto-typical floor plan for Two White Flint North (TWFNi office space. The typical plan for a floor in TWFN which is all office-space will accommodate just over 200 people, or about twice as many as the most denr.ely populated floor in One White Flint North (0WFN). However, there will be greater variation in layout and densities in TWFN than in OWFN because of i the greater number of special and support spaces to be incorporated. New Reg Cafe Replacement eouipment for the Cafe's beverage / cashier line was installed during the week. 1 Region I ltems of Interest i Week Ending September 28, 1990 1. Seabrook Commissioner Rogers and his legal assistant, M. Karman, toured the Seabrook site on Tuesday, 9/25. They were accompanied by M. Knapp, Director, Division of Radiation Safety and-Safeguards, Region I and the resident inspectors. Two intervenors also accompanied the Commissioner: Mr. Neil Morrissy, a Selectmen from Amesbury, Massachusetts; and Mr. Donald Beaulieu, Chairman of the Salibury, Massachusetts Selectmen.- 2. Washington Hospital Center, Washington, DC On September 18, 1990, Washington Hospital Center, Washington, D.C.,-informed the NRC Region I that on September 18, 1990, while a patient was undergoing external beam therapy with a cobalt-60 teletherapy machine (Theratron Model 780), the radiation source did not retract to its fully shielded position at the end of the prescribed patient treatment. The machine operator-(technologist) immediately removed 'the patient-from the teletherapy room. After removing the patient, the technologist successfully returned the source to the fully shielded position. The machine malfunction resulted in an additional treatment time of about 12 seconds and an additional dose to the patient of 19 rads. On September 19, 1990, a Confirmatory Action Letter (CAL) was issued by Region I with the understanding that the machine will not be used until the cause of the malfunction is identified, appropriate repairs completed and RI notified, in addition, the CAL required the licensee'to review and revise their emergency operating procedures to ensure' timely and appropriate action is taken to minimize patient and technologist exposures, and provide training on these procedures to include emphasizing the need to follow procedures. On September 24, 1990, the licensee notified Region'I that the cause of the problem was a malfunction of a solenoid valve in the pneumatic source drive system. All repairs were completed, the teletherapy machine was tested, procedures were reviewed, and training was conducted prior to the resumption of patient treatment. Region I notified the licensee that all issues identified in the CAL were satisfied. 3. Vermont Yankee - Safety System Functional Inspection (SSFI) A Safety System Functional Inspection conducted at Vermont Yankee August 6-17, 1990, identified a condition where the four Residual Heat Removal Service Water (RHRSW) pump motor cooler drain lines passed through a common-shortlengthofpipeandshutoffvalve(V999). Closure of the valve.would stop cooling water flow through the RHRSW pump motors. The safety analysis for a 1982 piping material change from carbon steel to copper resulted in a FSAR update showing the common discharge shutoff valve. On September 25, 1990, the licensee presented additional information to the staff on this issue, which included a statement that the condition.of a common shutoff valve was a prelicense, as constructed configuration that was not modified by the 1982 material change. The licensee does not consider this condition-to be a safety issue. Additional information including detailed plant-design change documents will be provided to the Region I staff for review. Carolina Power and Light Company On September 24, 1990, representatives from the Carolina Power and Light l Company were in the Region 11 Office to discuss their'self-assessment of performance at the Brunswick facility. 2. Florida Power and Light Company On September 24, 1990, representatives from the Florida Power and Light Company,;cre % the Region 11 Office to present their self-assessment of-performance at the St. Lucie facility. 3. Tennessee Valley Authority On September 24, 1990, representatives from the Tennessee Valley Authority were in the Region 11 Office to discuss ALARA program corrective actions at Sequoyah. - On September 25, the Regional Administrator was-in Headquarters to meet with various senior managers prior to the Browns Ferry Commission Briefing. 4. NRC-Sponsored Workshops On September 27, an NRC-sponsored workshop with. Region 11-licensees was held at the OMN! Hotel in Atlanta on the subject of "Backfitting." The Regional Administrator presented the opening remarks. On September 28, an NRC-sponsored workshop with Region Il licensees was held at the OMN1 Hotel in Atlanta on the subject of " Event Reporting The Regional Administrator presented the opening remarks. 5. H. S. Navy Radiation Safety Committee Meeting On September 25, 1990, the Director and Deputy Director, DRSS attended the quarterly V. S. Navy Radiation Safety Committee Meeting at the Navy's. offices in Crystal City, Virginia. The NRC representatives discussed the NRC enforcement policy, licensing actions proposed by 'the Navy, and the NRC inspection program. The NRC representatives agreed.to work with Navy representatives concerning license changes that the Navy may submit-to streamline the license. 6. Veterans Administration (VA) Safety Seminar A Region II Radiation Specialist participated with representatives from HMSS and Region IV in presentation of a national level radiation safety seminar for Veterans Administration RS0s, physicians, and materials users.- The seminar consisted of two separate two-day sessions (September 25-26 and September 28-29) held at the VA's national center in Little Rock, Arkansas. 7. Applied Radiation Energy Corp. (ARECO) Meeting On September 25, 1990, the Region II Branch Chief for Nuclear Materials Safety and Safeguards along with representatives from HQs-NMSS met with management representatives from ARECO in Lynchburg, Virginia to discuss contingency plans for removal of WESF capsules (Cs-137) from the firm's commercial irradiator facility. The licensee acknowledged NRC's concerns and indicated they would develop a schedule for removal. The licensee is planning to move to a new facility and change to use of Co-60 sources. The removal of the WESF capsules will be dependent on transportation cask availability and the' licensee's relocation to a new irradiator facility. 8. Brunswick N-16 Level Increase The licensee reported an increase in N-16 levels to the extent that radiation levels in a parking lot reached 0.8 mR/hr for a short period. During hydrogen injection, nitrates and nitrites normally seen in;the RCS are reduced to more volatile nitrogen compounds resulting in increased N-13/N-16 carryover into the MSLs. When the RWCU was isolated, contami-l nants were not being removed (chlorides, nitrites / nitrates and sulfates) and were available to form their acids which depressed the pH, creating additional H+ ions to increase nitrate / nitrite reduction to ammonia, resulting in additional carryover, and increased doses. When the RWCU'was placed back in service after one week, MSL radiation levels returned to i pre-isolation values. Brunswick plans to continue using H to reduce pipe cracking. They will continue monitoring radiation levels. 2 9. Florida Power Corportation On September 25, 1990, representatives of the Florida Power Corporation (FPC) Crystal River facility met with Region 11 and NRR to present a safety assessment of the lubricating oil leak on the "D" Reactor Coolant Pump. The utility plans a shutdown to Mode 3 on October 6, 1990, to allow a Reactor Building entry and assessment of the leak. Plans are to repair l the-leak and return the unit to operation by Monday, October 8,1991. l 10. Hatch Small-Scale Emeroency Exercise A small-scale emergency exercise was conducted on September 27, 1990 at Hatch. The Hatch exercise was successful with the exception of one i j weakness identified for failure to promptly detect an unplanned release.

11. Crystal River Small-Scale Emergency Exercise Crystal River conducted a small-scale emergency exercise on-September 27, 1990. The State of Florida participated for training only; their response was not evaluated by FEMA. The exercise was successful, but the licensee made very conservative protective action recommendations early in the-1 exercise, at Site Area Emergency.

These recommendations resulted in more comprehensive simulated offsite evacuations than the actual simulated emergency called for. l l SEPTEMBER 28, 1990 ENCLOSURE N i

? Region III Items of Interest Week Ending September 28, 1990 1. St. Louis Testing Labs. Inc. On September 25, 1990, C. E. Norelius, Director, Division of Radiation Safety and Safeguards, and selected regional staff conducted an Enforcement Conference to discuss the violations identified during a recent inspection. One of the violations included a personnel exposure slightly in excess of-the NRC limits; however, it was concluded that escalated enforcement'will not be taken. 2. West Shore Hospital, Manistee, Michigan On September 26, 1990, a Radiation Specialist and a Regional Management Representative were dispatched to the facility-in response to a reported significant diagnostic misadministration which occurred on September 22, 1990. The event involved the administration of 135 millicuries of a technetium-99m gallbladder imaging agent rather than the prescribed. dose _ _. of 7 millicuries of the radiopharmaceutical. Internal-dose estimates to the patient are 2.4 rads to the whole body and 58 rads'to the large intestine. The Region has discussed the event with'an NRC medical consultant and he is presently reviewing the case. 3. Quad Cities Nuclear Power Station On September 25, 1990, the Division of Reactor Safety conducted a management meeting with Commonwealth Edison Company (CECO) to discuss containment leak rate testing and CECO's actions to address identified deficiencies at the Quad Cities Nuclear Power Station. l 4. Monticello/ Prairie Island Nuclear Generating Stations Commissioner Curtiss visited Prairie Island and Monticello on September 24 and 25, 1990, respectively. He was accompanied by T. O. Martin, Deputy Director, Division of Reactor Safety, and the resident inspectors. i 5. Zion Power Station, Unit 2 OneoftwoUnit2maintransformersexplodedat9:51p.m.(CDT)on September 22, 1990, while the plant was starting up from a maintenance outage. The resulting fire was extinguished in about 20-minutes by the plant fire brigade. The Zion Fire Department was summoned for assistance. The licensee declared an Unusual Event under its emerg ncy plan. There has been local news media interest in the event. The reactor tripped from 40 percent power after a turbine trip occurred as. I a result of the transformer failure. All safety systems functioned as designed. There was no loss of offsite power. Unit 1, which was in operation, was not affected. 5. Zion Power Station, Unit 2 (continued) bus duct was also damaged. The licensee is preparing to inspect the-generator to determine if it was damaged. The cause of the event is still under revicw by the licensee. The licensee expects to have some results by the week of October 1. The licensee has a replacement transformer at another station which they will install. They estimate the' unit will be out of service for about 10 weeks. The licensee's inspection and repair activities will be monitored by the resident inspectors.- 6. U. S. Army, Rock Island, Illinois j On September 25, 1990, the Army notified Region Ill that luminous sights, each containing about 100 microcuries of promethium-147 on disposable rocket launchers, had been improperly dis)osed of at Fort Bragg, North Carolina. The initial report indicated t1at 600 to 1,000 sights were involved. The sights, mounted on the rocket launchers, are required by procedure to be removed prior to the disposal of the launchers. However, the Army reported that the sights had not been removed from the devices following their use. A Region 11 inspector was dispatched to Fort Bragg - -- - --~~~ on September 27, 1990, to investigate the circumstances o _the disposa1. f Further information indicates that ab'ouf 4,000 additional rocket ~ launchers had been crushed and buried at the site during the past nine months. The Army has agreed to recover the sights and preform any necessary decontamination work. Region 111 issued a Confirmatory Action Letter to - the Army, documenting its agreement'to fully investigate the disposal practices and to decontaminate the disposal _ area at Fort Bragg.- 7. Nuclear Non-Operating Owners' Group i Or. Carl J. Paperiello, Deputy Regional Administrator, spoke on i September 27, 1990, to a conference of the Nuclear Non-Operating Owners' 4 Group in Akron, Ohio. Also speaking were Commissioner Kenneth C.. On Seatet er 25, 1990, Entergy Operations, Inc., hosted a Plant Managers' Worksiop in New Orleans for Region IV plants to discuss a wide range of items of common interest. Five sites were represented, including Waterford 3. The meeting served to improve dialogue betreen plant managers 11 Region IV. This was the first meeting; however, future j meetings o' this type, with invitations for NRC Region IV representation, were discuised. 4. South Texa; Project Senior Management to Observe Plant Operations The Vice President of Nuclear Generation at the' South Texas Project notified Region IV management that senior plant management _ personnel would be observing plant operation on a 24-hour basis during the week of September ::0,1990. This action -is being taken in response to recent events involving personnel errors at the-site. The licensee indicated that 50 percent of the monitoring time will be' spent in the control room observing i:ontrol of work activities and communications. Below Regulatory Concern Public Meeting The Deputy Regional Administrator, accompanied by representatives of RES, NMSS, NRR and other Headquarters personnel, is conducting the Region V Below Regulatory Concern Public Heeting at the Holiday Inn in Oakland today. A press conference is being held after the Public Meeting. 2. Portland General Electric Company (Trojan) A Notice of Violation without a proposed civil penalty was sent to the licensee on September 21, 1990. The Notice cited failure of the established security measures to detect and prevent an individual carrying a loaded firearm in a briefcase through the security access building and into the plant protected area. The violation was categorized as Severity Level Ill. A civil penalty was not proposed because the licensee identified and reported the_Vinlation_to the_NRC and initiated a. comprehensive program of corrective actions to preclude recurrence of the violation. 3. Arizona Public Service Conpany (Palo Verde) A Notice of Violation and Proposed Imposition of Civil Penalty in the amount of $75,000 was sent to the licensee on September 26,'1990..The Notice cited the licensee's failure to (1) notify the NRC regarding licensed operators who had developed potentially incapacitating medical conditions; and (2) provide the NRC with accurate certifications that i candidates for operator licenses had received complete medical examinations. The violations were classified in the aggregate as a Severity Level 111 problem. The penalty was escalated 50% of the base civil penalty amount because the licensee's corrective action was limited. The results of a subsequent NRC inspection identified. additional examples of the violations. 4. San Onofre Nuclear Generating Station Units 1,2,3 Region V management and NRR representatives visited-the' San Onofre site on Septen.ber 24 and 25, 1990. A management meeting was held.to discuss issues of current interest;,-including the recently' completed single failure analysis for the Unit I safety injection system.
