I & E Inspection Report 83-29

REFERENCE:

NRC Letter dated November 15, 1983

Dear Mr. Collins:

This letter is in response to the Notice of Violation received as a result of inspections conducted at Fort St. Vrain during the period October 1-31, 1983. The following response to the items contained in the Notice of Violation is hereby submitted:

Failure to Follow Technical Specification Requirements Technical Specification SR 5.4.4, "PCRV Cooling Water System Temperature Scanner - Surveillance Requirement," states, "PCRV Cooling System temperature scanner readings shall be checked by comparison of representative liner cooling tube thermocouple outputs to their respective subheader temperatures and associated alarms tested once per month during power operation."

Contrary to the above, the NRC inspector determined that the alarms were not test for the month of August 1983, and that none of the above requirements were performed for the month of September 1983.

This is a Severity Level IV Violation.

! (1) The corrective steps which have been taken and the results achieved:

As indentified by the NRC inspector the alarms were not tested for the month of August 1983 and neither the alarms nor the scanner reading temperature comparisons were performed for the month of September.

However, the 46 System flow and temperature scan evaluation was performed during this time period on a manual basis.

This test is primarily a PCRV Cooling System performance test.

Upon notification by the NRC inspector two special tests (SR 5.4.4) were issued, one for the month of September and one for the month of October.

These tests were both completed by 10-06-83.

(2) Corrective steps which will be taken to avoid further violations:

A' new temperature scanner system has been installed temporarily to replace the old scanner and the alarm function has been made operable.

(3) The date when full compliance will be achieved:

Technical Specification compliance was achieved on 10-06-83.
