NRC INSPECTION REPORT 50-456/96016(DRS), 50-457/96016(DRS) AND

Dear Mr. Stanley:
On March 12,1997, the NRC completed an inspection of your Braidwood Nuclear Power
.
Plant, Units 1 and 2. The inspection included a review of Appendix R issues and the fire
protection program. At the conclusion of the inspection, the findings were discussed with
those members of your staff identified in the enclosed report.

Based on the results of this inspection, certain of your activities appeared to be in violation
i
of NRC requirements, as specified in the enclosed Notice of Violation (Notice). The
violation is of concern because required hot work permits had not been properly or
completely filled out from January to September 1996.
'
,
You are required to respond to this letter and should follow the instructions specified in the
i
enclosed Notice of V!o!dtion (Notice) when preparing your response. In your response, you
should document the specifi.: actions taken and any additional actions you plan to prevent
'
,
recurrence. After reviewing your response to this Notice, including your proposed
,
corrective actions and the results of future inspections, the NRC will determine whether
'
further NRC enforcement action is necessary to ensure compliance with NRC regulatory
[
requirements.
!
In addition, two apparent violations were identified and are being considered for escalated
[
i
enforcement action in accordance with the " General Statement of Policy and Procedure for
i
NRC Enforcement Actions" (Enforcement Policy), NUREG-1600. The first apparent
violation involved the failure of auxiliary building roll-up doors to satisfy the Appendix R
3-hour rated fire barrier requirement. The second apparent violation involved examples of
'
inadequate separation of cables associated with redundant safe shutdown trains.

Accordingly, no Notice of Violation is presently being issued for these inspection findings.
Please be advised that the number and characterization of apparent violations described in

the enclosed inspection report may change as a result of further NRC review.

A predecisional enforcement conference to discuss these apparent violations has been
scheduled for 10:00 a.m. (CDT) on April 23,1997, at the Region ll1 office. The
determined that a violation has occurred or that enforcement action will be taken. The
purposes of this conference are to discuss the apparent violations, their causes and safety

significance; to provide you the opportunity to point out any errors in our inspection report;

and to provide an opportunity for you to present your proposed corrective actions. In
particular, we expect you to address the control room heat up rate associated with the
loss of control room ventibtion and the results of your recent High Energy Line Break
(HELB) Analysis performed during your review of the roll-up door issue. In addition, this is
,
an opportunity for you to provide any information concerning your perspectives on 1) the
.
severity of the violations,2) the application of the factors that the NRC considers when it
determines the amount of a civil penalty that may be assessed in accordance with
,
i
Section VI.B.2 of the Enforcement Policy, and 3) any other application of the Enforcement
Policy to this case, including the exercise of discretion in accordance with Section Vll.
You will be advised by separate correspondence of the results of our deliberations on this
matter. No response regarding these apparent violations is required at this time.
,
