SUBJECT: NRC INSPECTION REPORT 50-483/98-18 AND NOTICE OF VIOLATION

Dear Mr. Randolph:

From Sept 7mber 14 through October 2,1998, a safety system engineering inspection and

inspection of your program to implement 10 CFR 50.59, " Changes, Tests and Experiments,"

wre conducted at your Callaway Plant reactor facility. Following the exit meeting, the results of

these inspections were discussed with Mr. Mark Reidmeyer of your staff. The enclosed report

presents the scope and results of these inspections.

The safety system engineering inspection included an assessment of the capability of the

essential service water system and supporting electrical systems. On the basis of the results of

our completed reviews, we concluded that the essential service water was capable of

performing its safety functions. We also concluded that your engineering program continued to

be good, although two violations were identified relative to maintaining a sound design basis for

your electrical distribution system and operating within the assumptions of your design, The

NRC has concluded that information regarding the reasons for the violations, the cocrective

actions taken and planned to correct the violations and prevent recurrence is already

adequately addressed on the docket in the enclosed report. Therefore, you are not required to

respond to this letter unless the description therein does not accurately reflect your corrective

actions or your position. In that case, or if you choose to provide additionalinformation, you

should follow the instructions specified in the enclosed Notice.

One unresolved item was also identified concerning design changes made to bypass torque

switches, originally installed in safety-related motor-operated valve control circuits to prevent

overtorque, during valve closure. This matter is discussed in Section E1.1.1 of the enclosed

inspection report. We are concerned that your weak link analysis was not performed with the

maximum expected voltage, and as a result, you may not have identified pressure boundary

parts that could be damaged. We are also concerned that you may not have fully evaluated

valves that had safety functions in both the closing and opening direction, particularly those that

are located in high temperature locations during accident conditions. As discussed, during the

exit, we understand that you have committed to perform additional review to confirm the

adequacy of your design in this area.

Appropriate procedural guidance was in place to implement the requirements of 10 CFR 50.59

and the safety evaluations that were reviewed properly docurnented that no unreviewed safety

questions existed.
