Dear Mr. Buhr:

This acknowledges receipt of your NRC Form 241 dated February 4,1999, and the $1,100.00 application fee, which you submitted to report proposed activities in NRC jurisdiction under the authority of the generallicense pursuant to 10 CFR 150.20. Your Agreement State License No. IL-01300-01 is recognized as valid for the proposed use of licensed material as described in this initial application not to exceed 180 days in calendar year 1999.

As a reminder, NRC Form 241's should be submitted at least three days prior to start of any work activities in Non-Agreement States. When it becomes necessary, as in the case of this current application, a verbal request for a waiver of the reporting requirements, and granting of such a waiver, is necessary prior to the conduct of any work in NRC Jurisdiction. This was explained to you during our phone conversation on February 10,1999, and shou!d be adhered to in future submittals.

Please refer to Page 4 of 10 of the attachment to your application where "NIPSCO"is listed as a work location with no associated address and a work date of 4/29/99. Complete work location information must be submitted to us prior to start of yourwork activities on April 29, 1999, or prior to any rescheduled work date, whichever occurs first.

Deletion of specific work locations or work sites, work site contacts, addition and/or deletion of dates of work, or changes in the equipment to be used when there is no change in the activity to be conducted or the radioactive material to be used are considered clarifications and as such, do not require a fee. However, a $200 fee is required whenever you revise the initial NRC Form 241 to include additionallocations of work; when you propose to use different radioactive materials; or you plan to perform additional work activities not previously approved by NRC.

When submitting clarifications of proposed dates and locations of work, please include the Location Reference Number assigned to a particular work location. This information can be found on the approved NRC Form 241, attached.

Please provide us a copy of your renewed Illinois license when received.
