TRANSFER OF SEALED SOURCE AND DEVICE REGISTRATION FROM NEW YORK TO THE NRC, CERTIFICATE NO. NR-0502-S-106-S
Dear Mr. Colon-Gonzalez:
This is a follow-up to the letter which Ms. Booma Venkataraman, U.S. Nuclear Regulatory Commission (NRC), sent to you, dated September 29, 2023, informing you that the NRC would issue new sealed source and device (SS&D) registration certificates to replace the ones which had been issued by the New York State Department of Health (NYSDOH).because the authority to issue SS&D registrations has been transferred from the NYSDOH to the NRC.
Please note that we have issued a new registration certificate NR-0502-S-106-S to replace NY-0502-S-106-S, dated July 23, 1986. A copy of the new registration is enclosed.
Please be advised that you must manufacture and distribute the product in accordance with the statements and representations contained in your application, with enclosures thereto, and the information set out in your registration certificate. As a general rule, you must request and obtain an amendment to the certificate before you make changes or modifications to the information submitted to obtain the certificate.
Please read over the registration certificate in its entirety and notify us immediately of any errors or omissions.
You are obligated to notify us promptly in writing should you decide to no longer manufacture, distribute, or offer service support for the product.
