Dear Evan,

I understand. But you need to explain this in writing, under signature, to us. Apparently you did that with the amendment for the other license but not with this license. Without this being in writing, under current date and signature, it still looks like you havent accounted for or addressed the previous changes. We cannot accept just an email to make it part of your official agency records as it lacks a signature.

Please send that to us ,as described in the yellow highlighted section in my signature block below, as additional information to Control No. 637060 and you may address it to my attention by name. We can then resume our review.

We recently added the same d/b/a to our primary license (21-00243-06), a copy of which I have attached for reference. In our letter request for that amendment (ML23171A946), our review of the document you reference below is articulated. While SNM-1432 does not regularly require the communication with other licensees, I requested this for consistency. Please let me know if this is sufficient explanation - if you need more documentation, I can engage our compliance team.

Ive been assigned your amendment request to the SNM-1432 license to change the name on the license and the iden ty of the physician responsible for the nuclear pacemakers.

Has this hospital undergone a change in ownership/control that has resulted in this name change? If so, please refer to NUREG 1556 Vol. 15, Rev. 1, including, but not limited to, Appendix E.
