.' Appendix A fl0TICE OF VIOLATI0ti University of California at Los Angeles Docket flo. 50-142 Los Angeles, California 90024 License flo. R-71 As a result of the inspection conducted on June 28 to June 30, 1932, and in accordance with the flRC Enforcement Policy, 47 FR 9987 (March 9,1982),
the following violation was identified:
Technical Specification Section VII.J. " Procedures," states: "All procedures and major changes thereto shall be reviewed and approved by the Director of the fluclear Energy Laboratory prior to being effective. Changes which do not change the original intent of a procedure may be approved in writing by the reactor supervisor.
Such changes shall be recorded and submitted to the Director for routine review. The following types of written procedures shall be maintained:
- 1. flormal startup, operation, and shutdown procedures for the reactor. These procedures shall include applicable check-off lists and instructions."
Contrary to the above on June 29, 1932, an addendum to " pre-start check-off" dated January 11, 1932, had not been submitted to the Director for routine review.
This is a Severity Level V Violation.
Pursuant to the provisions of 10 CFR 2.201, the University of California, Los Angeles is hereby required to sutnit to this office within twenty-five days of the date of this flotice, a written statement or explanation in reply, including: (1) the corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further items of noncompliance; and (3) the date when full compliance will be achieved.
