April 18, 1988 M. J. Ray LF SW 1055-C APRIL 14 HEETING WITH HSBII REPRESENTATIVES TO DISCUSS UPCOMING NRC ENFORCEMENT CONFERENCE ON I&M ,

The subject meeting was held with HSBII to' Esther pertinent f acts /information regarding preparation for the upcoming NRC enforcement conference. TVA's attendees, besides you and I, were Dick Parker, Rick Levi, and Walt Joest and HSBII representatives were Bill Higginbotham, Chuck Lyons and Chuck Ireland.

The meeting was requested by TVA via Dick Parker's April 8 memorandum to Bill Higginbotham.

HSBII was very open and cooperative and provided a number of documents and information which will be useful in preparing for the enforcement conference.

The following is a list of major points discussed in the meeting.

1. HSBII did an investigation into this issue but not from an I & H perspective. They focused on the extortion letter to try to find out who prepared the letter and secondly to investigate any question of work quality. Their September 16, 1985 letter tc TVA (W. E. Hason) documents that they (1) found no quality problems, (2) could not identify who wrote the extortion letter. In spite of this, they made certain management changes to ensure TVA confidence in HSBII was restored.
2. HSBII first learned of possible I & H allegations when NRC/0! interviewed their management in Atltnta in May 1986. In response to 01 questions during these interviews, HSBII said they were not aware of any I & H.
3. HSBII believes they sent letters to TVA documenting that they had problers with the "use as is" dispositioning of NCR 56-09.

Mr. Higginbotham will send us a copy of these letters.

4. HSBII does not now believe we meet ASHE Code requirements on WBN unit 1.
5. HSBII does not believe they have ever received any allegations of I & H before. They will check and get back to us. .

6. NCR 56-09 was signed off by the A!:I who noted "signed as directed by management." HSBII said this was not unusual and in fact, they encouraged ANI's to do this if they had a problem, or to let their Jupervisor sign off for them. HSBII will check for documentation of this in training or procedure documents.

7. As part of the steps taken by MSSII noted in item 1 above, management audits of ANI Daily Inspection records for quality issues and acceptability of disposition were conducted for about one year. M5BII wi.1 provide us with documentation of this.
3. Acceptance by H5311 of the "use as is" dispositioning (i.e., no hydro required) of NCR 56-09 appears to be based on mi s c onnun ic a t ion /mi sund e r s t a nd ing . The ANI's are not required to inspect 100% of welds during the hydro tests of the flued head penetrations to meet Code requirements. However, the certificate holder (TVA) wusst observe all welds during hydro. This point of who had to de 100%
