Dear Ms. Coker:

This is in response to your letter dated April 15, 1986, in which you requested, pursuant to the Freedom of Information Act (F0IA), records regarding locations of incidents involving damage to Kay-Ray gauging devices containing Cesium 137 and caused by molten steel coming in contact with the source housing.

When Linda Robinson telephoned you on April 21 for clarification of the scope of your request with regard to the above subject, you stated that you are interested only in subject records that pertain to licensees in the Agency's Region II and only records since 1979. In a search of pertinent files in light of the clarification of the scope of your request, the staff identified the records listed on the enclosed appendix. On May 8, 1986, Ms. Nina Toms telephoned you to inform you that the identified records are remotely related to your request but that you may find the records of some interest. We are placing copies of the records listed on the enclosed appendix at the NRC's Public Document Room (PDR),1717 H Street, NW, Washington, DC 20555, in file folder F0ia-86-288 in your name. In the event you may wish to obtain copies.

of any of these records, we are enclosing a notice that provides information about charges and procedures for obtaining records from the PDR.

Also, as Ms. Toms indicated during your conversation, because South Carolina is an Agreement State, source users are required to submit notification of incidents to the state, not to the NRC. You then indicated to Ms. Toms your intent to contact the State of South Carolina to further pursue the matter.
