Subject:

RECOVERY OF LOST RADIOGRAPHIC EXPOSURE DEVICE On June 8,1998, the licensee notified Region IV that it had recovered the radiographic exposure device reported as lost on March 25,1998; the exposure device had fallen overboard into the water from an offshore platform located approximately 70 miles off the coast of Louisiana.

The licensee reported that, during a diving operation conducted at 6:00 a.m. (CDT) on June 8 the diving team located and recovered the exposure device. The device appears to have no additional damage other than the o.:;inal handic f*ilure. The iridium-192 sealed source was locked in the shielded position and the crank-out assembly was intact. The device will be transported to the licensee's New Orleans field office on June 10 so that a more detailed inspection and analysis can be performed.

The state of Louisiana has been informed.
