1 J APPENDIX A NOTICE OF VIOLATION Gulf States Utilities Docket: 50-458 River Bend Station License: NPF-47 During an NRC inspection conducted on June 16 through July 31, 1987, one violation of NRC requirements was identified. The violation involved a failure of timely followup action to verify that specified corrective actions were completed. In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR 2, Appendix C (1987), the violation is listed below:

l Failure of Followup Action Criterion XV' of 10 CFR Part 50, Appendix B and the licensee's approved QA program requires that measures be established to assure that conditions adverse to quality are promptly identified and corrected.

Quality Assurance Directive (QAD) 16, " Corrective Action," requires appropriate departments to provide for timely followup reviews to verify that specified corrective action has been properly implemented and corrective action documentation has been closed out. l l

Contrary to the above, on June 30 and July 15, 1987, respectively it was found that specified corrective action for NRC Violation 458/8531-01 and Deviation 458/8622-02 had not been implemented. Timely followup reviews had not been conducted to verify that specified corrective action was properly implemented. Specifically, corrective action for Violation 458/8531-01 included revising certain surveillance test procedures by December 31, 1986, and this had not been accomplished as of 4 June 30, 1987. Also, corrective action for Deviation 458/8622-02 included a commitment that operation of the radioactive waste sludge tank sample collection system would commence at the end of 1986. This sample collection system was still not operable as of July 15, 1987.

This is a Severity Level IV violation. (Supplements.D.)(458/8716-01)

Pursuant to the provisions of 10 CFR 2.201, Gulf States Utilities is hereby required to submit a written statement or explanation to the U.S. Nuclear Regulatory Commission, ATTN: Document Control Desk, Washington, D.C. 20555 with a copy to the Regional Administrator, Region IV, and if applicable, a ccpy to the NRC Resident Inspector, within 30 days of the date of the letter transmitting this Notice. This reply, should be clearly marked as a " Reply to a Notice of Violation" and should iaclude for each violation: (1) the reason for the violation if admitted, (2) the corrective steps that have been taken and the results achieved, (3) the corrective steps that will be taken to avoid further violations, and (4) the date when full ccmpliance will be achieved. If an adequate reply is not received within the time specified in this Notice, an order may be issued to show cause why the license should not be modified, h NK B G
