\'***** v(/ Transportation Certification Branch Acoroval Record Model No. E-tfEH-00-00004 Package Docket No. 71-9011 By application dated Parch 12, 1986, as supplemented April 25, 1986, Advanced Medical Systems, Inc. requested renewal of Certificate of Compliance No. 9011. There have been no changes requested or nade to the overpack or teletherapy head since approval of the application for renewal of the certificate of canpliance dated November 19, 1980.

The drawings of the special form material have been deleted fran the drawing list (Condition 5.(a)(3)). The requirement for sealed sources to neet the performance standards for special form material are cited in Condition 5.(b)(1) of the certificate.

The applicant should note that a special form encapsulation either designed or constructed after June 30, 1985 must neet the requirenents of the present regu'ations as given in 1.0 CFR Part 71.

The drawing list, Condition 5.(a)(3) of the certificate, has been updated to include the revision level of each drawing and to also include the drawing of the package identification olate.

The staff concludes that the statements of the original application satisfy the reouirement for renewal of the certificate of compliance.
