ATTACHfiFNT A Docket No. 50-460 Potentially Reportablo Condition Per 10CFR50.55Ce)

Boston insulated Wire S Cable Co.

RG-Il A/U Triaxial RG-22 B/U Twinaxini RG-58 C/U Coaxlal RG-174/U Coaxial DFSCRIPTION OF DEFICIENCY The Supply System purchased a nmber of different specialty control cables frcm f30ston Insulated Wire & Cable Co. (BlW) for control applications within WNP-1.

Contract requiroments includod supply of qualification documentatlon to IEEE 323-1974 and IEEE 383-1974 Prior to delivery of the specified quallfication documentation, the manufacturer provided a certificate of conpliance on each cabic type. Upon review of qualI fIcatIon documntatIon nutmltted by the manufacturer, It was establ1shed that the requirenonts of IEEE 383 had not been met.

ANAL.YSIS OF SAFETY liiPl.! CATION While the Inck of qualification doctrient at ion is not in itself evidence of potentini failure, unacceptable perfon, ance of tbn subject cable could result in potential loss of safety related Instnrvnt control signals.

CAU9F OF DFFICIFNCY The rnnufacturor's Interprotntion of IEEE 383 Table i permpted qualification by similarity and extrapointion from test reports on other products. Tablo I requ i rermnts for conxini and trinxial specinIty cables specifically prescribe typo tosts on nctual sizo nnd catngory of cable samples.

COPRECTIVF ACTION Bechtoi nonconformanco report 1-PNCR-106-14 has been written to documnt the identified potentially deficient cablo. At the present t irne, n finnt detonninat ion has not been nudo as to tho disposition of the cable. Two pov.lbilit ies currently beinq considered nro " reject" nnd roturn tho enble to the Vendor or " r < wo r k" and havo the vervfor porform addltIonal qualIflentton testing. Upon f i nn t d i spos i t i on o f t he not ed RNCR , nn updated 50.55(o) report will be provided.

ACTION TO PPEVENT PFCtlRPFNCE Evaluation of all documntat lon regul red to bo sutmitted by the contractor is perfonmd prior to closcout of n11 cont rac t e, . The dolay which occurred in this case was due to thn deferred construction activltles on tho WNP-1 Project. No further corrective action In required, sinco review would have been porforred in o rnro t irvl y fashlen if the Pro ject was in nctivo construction.
