It was not clear from Forms 10A and 14A that the radiographic exposure
device or storage container used at each job oite could be identified. This
is a requirement of 10 CFR 34.27(a). You may wish to add an item to Form 10A
and/or Form 14A. *
Since 10 CFR 34.11(d) requires quarterly personnel audits, the license
condition which referred to personnel audits has been deleted from your
license.
Any future correspondence relating to your license should specifically
reference your license and docket numbers to expedite your inquiry.
