This preliminary notification constitutes EARLY notice of an event of POSSIBLE safety or public interest significance. The information presented is as initially received without verification or evaluation and is basically all that is known by IE staff on this date.

TALSIFICATION OF TEST RECORDS INVOLVING CONCRETE AIR'ENTRAINME

The Texas Utilities Generating Company (TUCCO) informed NRC's Region IV '

office of an incident involving the falsification of concrete air entrain-ment test records for the Comanche Peak Project by an R. W. Hunt Testing Laboratories employee.

According to the TUGC0 QA manager, an R. W. Hunt concrete inspector assigned to testing concrete at the point of placement at the safeguards building is alleged to have initially recorded an air entrainment test' reading of 3.9%. However, after the placement, he realized the air entrainment was not within the specified 5.0% 1% limit and subsequently modified the pL aanent record to indicate a 4.3% reading.

The concrete in question was placed on January 20, 1977. The revised test records were identified by R. W. Hunt Laboratories d the inspector alleged to have falsified the records was discharged on Fe uary 8, 1977.

TUGC0 learned of this occurrence through their Constructors, Brown and ,

Root Company, on February 23, 1977. .

l, The TUCCO QA manager further tated that a detailed investigation is

currently underway by both TUGC0 and Brown & Root. An evaluation of g the concrete will be made during this investigation to determine its 1 i acceptability.

Region IV (Arlington, Texas) will investigate the matter during a site inspection scheduled for the week of February 28.

There has been no media interest to present and none is anticipated. I Region IV received notification of this occurrence by telephone from TUGCO' QA manager at 3:30 p.m. on February 25, 1977. The information is current as of 9:00 a.m. February 28, 1977.
