SUBJECT: NRC INTEGRATED INSPECTION REPORT 50 327/97 06 AND 50-328/97 06
Dear Mr. Kingsley:
This refers to the inspection conducted on May 25 through July 5, 1997 at the
Sequoyah facility. The enclosed report presents the results of this
inspection.
During the inspection period, your conduct of activities at the Sequoyah
facility were generally characterized by adequate operations, with good
control of power changes and problem identification and negative observations ,
in procedure compliance and o>erator training; adequate maintenance, with
positive observations in trou)leshooting and pre job briefings and negative
observations in procedure compliance and numerous equipment failures: adequate
engineering, including good quality of modifications and negative observations
in the Generic Letter (GL) 89-10 program, ASME Section XI program, and
corrective actions for deficient relief valve settings; and good plant
support, with positive observations in radioactive shipping, waste storage,
dose control, and water chemistry and a negative finding for failure to
maintain proper air pressure in the auxiliary building. Nuclear Assurance
audits were strong, intrusively focused and continued to identify significant
areas needing improvement. A significant finding by Nuclear Assurance
involved an observation that control room personnel generally reacted to
abnormal conditions in a routine manner. This is consistent with ongoing NRC
concerns that material condition deficiencies are numerous and often challenge
operators. The NRC is concerned with the findings identified during the
Corrective Action Program inspection. While it was noted tnat management
involvement in the Corrective Action Program has improved, weaknesses
identified during the ins)ection indicated that the improvements in /
implementing the process lave leveled off and continued attention is I
warranted.
Based on the results of this inspection, the NRC has determined that
violations of NRC requirements occurred. The violations are cited in the
enclosed Notice of Violation (Notice) and the circumstances surrounding them
l are described in detail in the subject inspection report. A violation was
! identified for inadequate corrective actions associated with improper safety
injection system relief valve settings. Following initial NRC concerns with
the interpretation of ASME code requirements and subsecuent corporate review
of the relief valve setpoint issue, the licensee failec to properly
