REVIEW OF MANUAL VALVE FAILURES The enclosed Technied Review of Manual Valve Failures is forwarded for your y information and consideration. l i -

Manur' valve failure data were obtained using the Nuclear Documents System (NUDOCS), sequence coding and search system (SCSS) and the nuclear plant reliability data system (NPRDS). The data obtained from NUDOCS and SCSS were not complete due to limitations in search strategies available. NPRDS provided a lar; amount of data, mostly about mechanical failures. To the extent oossible, human related failures

were obtained with NUDOCS.

Using NPRDS, approximately 6200 failures were retrieved including 5 events considered important using AEOD screening criteria. Most of the NPRDS failures, wi*h a' few exceptions, were routine failures. Details of the review and evaluations are included in this report.

There does not appear to be a systematic or pervasive problem associated w ith r.canual valves. Therefore, additional < .dy does not seem warranted at this time.
