INVESTIGATION NOTIFICATION The following information is submitted in compliance with the Comission directive that you be informed whenever the Office of Investigations (01) opens an investigation:
Case No. 1-90-018 Opened: 12/06/90 Yeterans Administration Medical Center ,
By memorandum dated December 3,1990, the Regional Administrator, Region I, requested that OI initiate an investigation at the Veterans Administration Medical Center, Albany, New York, to determine if radioactive sealed source inventory records, dated October 14, 1990, were deliberately falsified by the Acting RSO, and the extent of licensee management and the RSO's involvement / knowledge of this falsification. ECD: 03/91.

SUBJECT:
REQUEST FOR INVESTIGATION . FALSIFICATION OF QUARTERLY SEALED SOURCE INVENTORY, VETERANS ADMINISTRATION MEDICAL-CENTER (VAMC)(CASENO. 1-90-018)
On December 6,1990, Christopher Eckert, Health Physicist, Nuclear Materials Safety Branch, Region I, NRC, was contacted by 01 Edward Fitzgerald concerning his inspection of VAMC, Albany, New York, on November 20 and 21,1990.
Ecker t said that during his review of the sealed source inventories, he selected the October 1990 inventory and asked Dr. Andrews Kang, the RSO of record, if the inventory was done, Kang said that-it was and that some of the scurces were difficult to locate. Eckert asked a Dr. James Zhang, the acting RSO, about inventory and Zhang told Eckert that he (Zhang) conducted an inventory during which some sources were not found, and told Eckert that he bsed an old inventory listing. Eckert said that Dr. Kang signed all of the 1990 inventories on file indicating that they were completed.
