REQUEST FOR RENEWAL OF LICENSE NLHBER 48-12016-01 DATED April 23, 1984 AND OUR REQUEST FOR ADDITIONAL INFORMATION DATED May 30, 1986 Gentlemen:
We requested in the above mentioned letter that you respond to us within 30 days. A check of our files indicate that we have not received a response from you to date.
You are hereby notified that you have 30 days in which to submit a response to this notice.
Upon failure to file an answer within the specified time, it may be necessary to deny your application and tenninate your license. Such action would require that you divest yourself of all licensed material. Furthermore, our Inspection and Enforcement staff will be notified of the termination of your license.
Please respond in duplicate and refer to Control Number 17473.
