We have reviewed your letter dated November 5,1974, and the following is our response to the apparent violation cited in that report. This response (G) v' details the violation, corrective action and the results achieved, steps taken to avoid future violations, and the date when compliance was achieved or will be achieved.

During the routine inspection of October 1-4, 1974, the principal inspector reviewed the Bypass and Jumper Log. Apparent inconsistencies in the maintenance of the log and its governing procedure, 1005.04, " Control of Bypasses and Jumpers", were noted.

As corrective action, the Operations staff was made aware of the need for an immediate review and correction of the noted discrepancies. A follow-up inspection was done by QC personnel on October 24, 1974. Discrepancies as noted in the comments of the principal inspector had been corrected at that time. No further discrepancies were evident.

To prevent future discrepancies, the plant operations staff has been informed of the need for a continued effort to maintain this log as required by the procedure. Also, a QC inspection of this log is scheduled every four (4) months, or more frequently as necessary to assure compliance to procedures.

As of October 24, 1974, all noted discrepancies had been corrected.

-During'a subsequent inspection on November 20-22, 1974, the AEC Principal Inspector questioned the whereabouts of pages 31 and 48 of the Bypass and Jumper Log, since they were not readily available in the log.

A follow-up search for these pages revealed that page number 1 was also unaccounted for.

A visual inspection of all safety related cabinets anc' other areas where bypasses and jumpers might be located was initiated. No bypasses or jumpers were noted that were not accounted for on other log sheets, therefore these sheets of the log are assumed to have been lost or destroyed. The plant file will reflect-our findings in this matter.
