NON-RADIOLOGICAL NPDES PERMIT VIOLATION REPORTS Section 3.2 of Appendix B, Environmental Protection Plan (Non-Radiological) to Provisional Operating License No. DPR-20, entitled, "Reporting Related to NPDES Permits and State Certifications" states that copies of reports to the State of Michigan indicating violations of NPDES permits are to be sent to the NRC. Attached to this is a copy of one NPDES violation covering the six (6) month period from April 1987 through October 1987.

Parameter: Oil and Grease Parameter Code No: 00556

1. Date of Incident: August 17, 1987 (1030 Hours)
2. Identification of specific ~£fluent limitation or monitoring requirement affected:

Exceeded daily maximum limit 20 mg/L oil and grease

3. Description of Incident:

A routine sample of the turbine sump oil separator tank (T-41) on 8/17/87 -

(1030 hours0.0119 days <br />0.286 hours <br />0.0017 weeks <br />3.91915e-4 months <br />) was reported on 8/20/87 to be 83.5 mg/L.

4. Apparent Cause:

On 8/16/87, a spill of EHC (electrol hydraulic control) fluid (synthetic oil) resulted from a ruptured EHC line which caused an increase of oil present in T-41. A second sample was taken on 8/20/87 to ensure the parameter was returned to normal.

5. If not immediately corrected, the anticipated time the'condition is expect-ed to continue:

6. Corrective Action - The following corrective action was taken or will be taken to reduce, eliminate and prevent recurrence:

Appropriate EHC lines were replaced to protect against future ruptures.

Turbine sump oil separator, M:l8, was pumped out the system, cleaned, and excess oil removed on 8/17/87.

7. Additional Comments:

Visual ~bservations of the mixing basin and Lake Michigan on 8/17/87 showed no oil present.
