ASAP GRANT FUNDS REIMBURSEMENT SYS US TREASURY Period of Performance: 08/31/2015 to 08/29/2019
NRC-HQ-7P-15-G-0006 M0005 The purpose of this modification is to:
- 1. Provide incremental funding;
- 2. Extend the period of performance; and
- 3. Revise the Federal Financial Report (FFR) and Performance Progress Report (PPR) reporting requirements.
As a result of this modification:
- 1. Funding in the amount of $39,904.40 has been provided thereby increasing the total obligated amount by $39,904.40 from 359,649.60 to $399,554.00.
- 2. Revise ATTACHMENT A - SCHEDULE (i) A.2, A.GENERAL, item 2, delete in its entirety and replace with the following:
"Total Obligated Amount: $399,554.00" (ii) A.4 AMOUNT OF AWARD AND PAYMENT PROCEDURES, item 2, delete in its entirety and replace with the following:
"2. Funding in the amount of $399,554.00 has been obligated for payment for the period specified in Section A.2.2. Additional funding for this award is contingent upon the availability of appropriated funds, satisfactory performance, and the recipients capacity to manage the award and comply with award requirements. Any work performed by the grantee beyond the funding amount obligated in Section A.2.2 will be at the grantees risk."
- 3. The period of performance has been extended for one year at no additional cost to the government. The period of performance has been extended to 08/29/2019.
- 4. The FFR and PPR reporting requirements found in Attachment C - Standard Terms and Conditions have been revised as follows:
FEDERAL FINANCIAL REPORTS Federal Financial Reports (SF-425) are semi-annually, for the periods ending March 31 and September 30, or any portion thereof, regardless of the award date. Reports are due within 30 calendar days following the end of the reporting period and must be submitted through FedConnect. The SF-425 form and instructions are available at Grants.gov.
PERFORMANCE PROGRESS REPORTS The performance (technical) progress report indicated below is subject to 2 CFR §200.328.
Performance reports must be submitted semi-annually, for the periods ending March 31 and September 30, or any portion thereof, regardless of the award date. Reports are due within 30 days following the end of each reporting period and must be submitted through FedConnect.

Final Reports - The Recipient is required to submit final reports, both Financial (SF-425) and Performance (SF-PPR, SF-PPR-B, SF-PPR-E) within 90 days of the grant expiration. In addition to these reports, a final SF-428, Tangible property report, is also required, if applicable.
