Date of Occurrence 5/8/81 Time of Occurrence 1550 Unit 1, 2, and 3 Identification and Description of Occurrence:
Power (120V AC preferred) lifted to fire pump auto-initiation. The exact duration the circuit had been inoperable could not be determined. However, it is probable that the wiring was removed during the modification activity, making the duration 2 weeks ur less.
Conditions Prior to Occurrence:
Unit 1 refueling outage.
Unit 2 at 99%.
Unit 3 at 100%.
Action specified in the Technical Specification Surveillance Requirements met due to inoperable equipment. Describe.
Reterminated power supply.
Apparent Cause of Occurrence:
Wiring was removed from terminal points in main relay panel 52 resulting in the interruption of power to the fire pump auto-start circuit. Although it could not definitely be determined how the wiring was removed, it is suspected that the wires were removed during performance of a molification in tne panel.
Analysis of Occurrence:
There was no danger to the health or safety of the public, no release of activity, no damage to the plant or equipment, and n) resulting significant chain of events.

Replaced permanent wiring immediately. Precautions have been added to the l workplan requiring the electricians to consu'; the cognizanc engineer prior to removing any existing wiring.
