Dear Mr. Vanderpool:
This letter is in response to the Hopewell Designs, Inc. request dated April 3, 2018, requesting a sealed source and device registration certificate for the device model SG-101010. We have completed your request. A copy of the registration certificate is enclosed.
Please be advised that you must manufacture and distribute any registered product in accordance with the statements and representations contained in your application, with enclosures thereto, and the information set out in your registration certificate. A registration certificate is meant to indicate that your product is acceptable for licensing purposes but does not authorized distribution or possession of radioactive material. As a general rule, you must request and obtain an amendment to the certificate before you make changes or modifications to the information submitted to obtain the certificate.
Please note that the information submitted to the U.S. Nuclear Regulatory Commission by a licensee requesting evaluation of a sealed source and device should contain all the required information at the time of submittal. Applications containing incomplete and/or inaccurate information will not be accepted for review and may be denied in the future.
Please read over the enclosed registration certificate in its entirety and notify us immediately of any errors or omissions.
You are obligated to notify us promptly in writing should you decide to no longer manufacture, distribute, or offer service support for the product.
