The licensee discovered this event on October 15, 2016 and notified the Department [PA Department of Radiation Protection] on October 15, 2016 via voice message after business hours. It is reportable within 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> per 10 CFR 30.50(b)(2).
During normal daily operations, a density gauge was found to have a shutter that was nonresponsive and stuck in the open position. No overexposures have occurred and the gauge has been taken out of service. A service provider has already made the necessary repairs as of today, October 17, 2016. The gauge is an Ohmart/Vega Model SHLG. The serial number of the device is unknown at this time. The device originally contained 1200 millicuries of Cs-137.
The cause of the event is unknown at this time.
A reactive inspection is planned by the Department. More information will be provided upon receipt.
